Job Description - Community Liaison - Skilled Home Care

Job Title: Strategic Referral Partner – Home Health Sales Executive

Company: Home Health Care, Inc.

Location: Greater Twin Cities Metro Area (21 Counties)

Salary: $90,000 to $120,000 Base + UNLIMITED COMMISSION

Schedule: Full -Time | Monday – Friday

Your Impact:

  • Be the Face of Home Health Care, Inc.: Represent our brand at hospitals, clinics, senior living facilities, and community events.
  • Drive Referral Growth: Develop partnerships with discharge planners, case managers, physicians, and other key sources.
  • Educate and Inspire: Present the value of home care through compelling education and outreach.
  • Coordinate with Clinical Teams: Align sales strategies with intake, field staff, and leadership for a seamless experience.
  • Analyze and Improve: Track performance, adjust outreach, and find new growth opportunities.

What We’re Looking For:

  • Bachelor’s Degree in Marketing, Communications, Business, or related field preferred
  • 2+ years in healthcare sales, preferably in home care, hospice, or senior services
  • Excellent communication and relationship -building skills
  • Proactive self -starter who thrives with independence and collaboration
  • Knowledge of Twin Cities healthcare landscape a strong plus
