Job Description - Community Mental Health Provider

Description

Venture Forthe is a leading health care agency in Central and Western New York that operates on the fundamental premise that every individual has the right to make their own choices; we advocate for the needs and goals of our clients. The clinical department at Venture Forthe focuses on HCBS (Home and Community Based Services)/CORE (Community Orientation Recovery and Empowerment Services) programs under the direction of the Office of Mental Health.


The goal of the CORE/HCBS program derives around the focus of increased behavioral health and addiction recovery through an outpatient system. This includes providing clients with the tools, resources, and skills they can utilize to help in their recovery journey. We can proudly say that we have helped our clients reach milestones such as obtaining a higher education degree, grow relationships with friend and family members and regain the ability to be fully independent in the community.


The Community Mental Health Provider will be involved in recovery-oriented activities and interventions that support and restore social and interpersonal skills necessary to help individuals reach their goals during their recovery. The Community Mental Health Provider will help increase or sustain community tenure, enhance interpersonal skills, establish support networks, increase community awareness, develop coping strategies and effective functioning in the individual’s social environment such as home, work, and school. The Community Mental Health provider assist support an individual with developing social skills (i.e., conversation and listening skills), wellness (i.e., meal planning), and personal care (i.e., managing finances).


POSITION SUMMARY:

  • Provide HCBS and CORE services for clients as needed in person and remotely
  • Provide individual clinical supervision including case consultation to address individuals with increasing psychiatric needs, referral to alternative services, risk management, and addressing any other health and safety concerns.
  • Ensuring regular availability and check-in with a clinical supervisor supports quality care, staff morale and retention, and overall professional development.
  • This consultation may also be provided when an individual is not making expected progress and the direct service staff requires guidance and support to adjust their interventions.
  • Responsible for approval and monitoring clinical department documentation processes
  • Complete approval or guide staff on revisions for individual service plans
  • Coordinate with staff support team to identify opportunities to improve staff documentation and outline specific plans with timelines / monitoring
  • Collaborate on complaint & incidents with department team
  • Maintain tracking process and related documentation for all complaints and incidents reported to the Clinical Department
  • Assist with investigation of complaints or incidents as assigned by Clinical Leads
  • Coordinate communication with clinical leads to VF departments such as HR or Compliance or external organizations
  • Assisting team in the development of workflows, documentation standards and inner agency standards/policies.
  • Complete LPHA recommendations for new clients and document required information to start CORE services. Coordinate with managed care organizations and CORE outreach team to establish contact with clients to determine CORE services and submit documentation.

Requirements

Required travel: 100%

  • Rehabilitation counseling to support and restore social and interpersonal skills necessary to increase or sustain community tenure, establish support networks, increase community awareness, develop coping strategies, and effective functioning in the individual’s social environment such as home, work, and school.
  • Rehabilitation counseling including recovery activities, interventions, and support necessary for the individual to implement learned skills so the person can remain in a natural community location.
  • Assisting the individual with effectively learning adaptive behaviors responding to or avoiding identified precursors such as cravings or triggers that result in relapse or functional impairments.
  • All components include additional job duties as assigned

QUALIFICATIONS:

  • Must possess a high school diploma or GED
  • One year experience supporting people in the Mental Health field or Substance Abuse recovery
  • Must have an active driver's license and access to private reliable transportation
  • Knowledge in the following areas is preferred: disability accommodations and assistive technologies and services, DSS/SSI, Transportation and Housing Access, and Community SUD resources.
