Job Description - Community Outreach Coordinator

Overview

Chestnut Health Systems is seeking a full-time Community Outreach Coordinator to serve on the Statewide Recovery Oriented System of Care (ROSC) Project. Based out of our Bloomington, Il location, successful candidate with work hybrid from office, home, and service area, with expectation to travel within the state.

We provide fully integrated care for all of our patients by combining behavioral health care services with our community-based primary care health centers. Responsibilities

Oversee development, implementation, and evaluation of programs/ services.
Participate and assist in coordination of efforts related to assigned IDHS- SUPR State ROSC leadership Center.
Oversee development, implementation, and analysis of process and outcome evaluations and community needs assessments.
Oversee development, implementation, and evaluation of media releases.
Oversee development and submission of grant and contract work plans according to project deliverables.
Oversee development and management of grant budgets.
Oversee development and timely submission of program/grant reports.
Hire, train, ensure competency of, mentor and supervise staff and interns.
Participate and take leadership role in local, state and national advocacy groups/collaborations.
Provide direct service as it relates to the implementation of programs as assigned by supervisor.
Fulfill agency responsibilities including staff development, monitoring research, participation in agency meetings and committees and other responsibilities as designated by supervisor.
Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness and spirituality/connectedness in all interactions with others, including utilizing recovery language.
Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.
Perform other duties as assigned.

Qualifications

Minimum of a master’s degree in education, public health, behavioral health related field or other field related to grant deliverables, or bachelor’s degree in education, public health, behavioral health or other field related to grant deliverables with at least five (5) years of related experience.
Requires advanced organizational skills and verbal and written communication skills.
Knowledge, skills and understanding of continuum of care, including understanding of prevention, intervention, treatment, and recovery services preferred.
Must have experience in coordinating community-based activities.
Knowledge, skills, and understanding of best practices, curriculum development and community coalitions required.
One year’s supervisory experience preferred.
Satisfactory completion of Chestnut's background checking process.
