Community Outreach Educator

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois.  We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility.  We also have a 24-bed inpatient unit in Joliet.  Non-clinical staff are based out of our Joliet administrative offices.

Community Outreach Educator

The Community Outreach Educator serves as an ambassador for Lightways by providing education and building community partnerships to increase awareness and access to hospice and palliative care. This role collaborates with healthcare providers, community organizations, and internal teams to reduce barriers to care, support diverse communities, and strengthen community engagement.

Key Responsibilities:

• Assess community education needs, with a focus on underserved populations.


• Provide hospice and palliative care education to community and healthcare partners.


• Partner with hospitals and health departments to support health equity and culturally responsive resources.


• Coordinate CEU programs with Business Development and Clinical Education.


• Build community partnerships to expand hospice and palliative care outreach.


• Represent Lightways at community events, health fairs, and professional conferences.


• Support initiatives that expand access to hospice and palliative care.


• Track community education activities and attendance.


• Maintain professional development through training, education, and staff meetings.


• Follow all applicable laws, regulations, and Lightways policies and procedures.


• Maintain regular and punctual attendance.


• Effectively use required organizational technology and communication systems.

Education/Experience:

• Bachelor’s and/or master’s degree in a related area of work – Marketing, Communications, Health Administration, Community Development.


• Minimum 2–5 years of experience in hospice, palliative care, health equity, or related healthcare setting.


• Demonstrated experience in staff education, training, or professional development.


• Excellent presentation, communication, and facilitation skills with diverse audiences.


• Ability to build collaborative relationships with community partners and healthcare professionals.


• Commitment to cultural humility, diversity, equity, and inclusion in care delivery.

• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
