Job Description - Community Outreach Navigator

Southwest Behavioral & Health Services is one of Arizona’s oldest and largest non-profit, community-based providers of behavioral health services. We offer an array of programs to meet the unique needs of the communities we serve. SB&H is a well-established voice in the community with more than 115 sites, over one thousand employees, and 40 years’ experience.


We are seeking a Community Outreach Navigator to join our team at the Avondale Resource Center. 


The Community Outreach Navigator will work in office Monday-Friday, standard business hours (no evenings, weekends or holidays).


Job Preview at a Glance:


The Community Outreach Navigator serves as a central support with a wide range of ages. This staff member will provide crisis intervention, one-on-one support, psycho-social prevention education and trauma-informed training to staff. This staff member is responsible for assisting individuals to develop and implement their personal wellness program by providing support, education, and motivation.


Responsibilities: 



  • Provides one-on-one community-based support to a caseload of members

  • Leads psycho-social prevention & wellness groups

  • Assists with crisis intervention with walk-in visitors

  • Administers trauma-informed training to team at Resource Center 

  • Works collaboratively with team to support individual service plans

  • Provides accurate documentation of contact with members



    Qualifications:



    • Minimum of 21-year of age

    • Bachelor’s degree in related field plus one year of relevant experience

    • Preferred: Personal Training Certification

    • Valid AZ driver’s license, and Motor Vehicle clearance

    • Ability to obtain fingerprint clearance card
