We are expanding our community-based outreach and are looking for motivated, well-connected, and heart-led individuals to help families access the in-home care and support they deserve.

Job Summary

As a Community Outreach Representative, you'll serve as the face of Your Tailor Made Senior Service in neighborhoods, senior communities, churches, housing developments, and local organizations. Your role is to promote our non-medical home care services—including assistance with daily living, companionship, light housekeeping, respite care, and more—and refer seniors and families who may benefit from our customized care.

Key Responsibilities

• Represent Your Tailor Made Senior Service Home Health at community events, health fairs, churches, senior living centers, and networking groups

• Promote awareness of our in-home care services, including personal care, caregiver support, and light housekeeping

• Identify and refer seniors and family caregivers in need of home care assistance

• Build meaningful relationships with community leaders, clergy, healthcare professionals, and senior advocates

• Maintain regular communication with our outreach and intake teams

• Report outreach activities, leads, and successful referrals

• Represent our brand with professionalism, empathy, and integrity

Ideal Candidate Profile

• Deep ties to the local community (e.g., caregiver advocate, church leader, senior center volunteer, social connector)

• Excellent communication and people skills

• Previous experience in community outreach, marketing, senior care, or social services (preferred but not required)

• Comfortable speaking with seniors, families, and referral sources

• Self-motivated and able to work independently

• Reliable transportation for local travel

• Passion for helping older adults live safely and comfortably at home
