$23.00 - $28.00 per hour

I. GENERAL DESCRIPTION

Under the supervision of the Outreach Manager, this position will focus on community outreach/education and event planning highlighting Imperial Beach Community Center and its programs and funding sources. Must be able to work in the evening and weekends. 

II. SPECIFIC RESPONSIBILITIES

Develop and maintain a thorough working knowledge of Imperial Beach Community Clinic services, provider staff and funding programs.

Identifies/research outreach locations for presentations.

Liaisons with community agencies to develop an outreach calendar.

Achieves financial results through the implementation of the outreach plan. 

Provides outreach presentations and distributes patient recruitment materials (i.e. brochures, fliers) at community locations and to individuals in the community to increase patient volume and health center revenue.

Seeks to actively engage community members at outreach events. Documents interactions

Counsels’ community members on their healthcare options (including Medi-Cal, Covered CA, Medicare), including the provision of health insurance screenings via the web-based screening tool. Inform community members and groups about the availability of and program guidelines for the following “health insurance” programs for low-income individuals:

-Medi-Cal and Medi-Cal Managed Care Plans

-Healthy Families

-CMS

-CHDP

-SOFP

-Title X Male Reproductive Health Services

-Self Pay Prenatal Care Programs

-BCEDP

Refers community members to Enrollment specialists. Making Appointments when necessary.

Utilizes a database system to enter and track outreach activities including the number of contacts.

Attending conferences may be required.

Refer appropriate individuals to Imperial Beach Health Center for necessary medical services – including assisting them in making their first appointment, if necessary.

Establish a communication network in the local community and represent Imperial Beach Health Center and its programs to the community through group and individual presentations. (community, church, business, neighborhood, and school groups).

Maintain appropriate logs of all activities.

Other duties as assigned.

III. QUALIFICATIONS

• Knowledge of local community/ socially connected to the communities of Imperial Beach and South San Diego.
• Able to provide services weekdays, weekends, and evenings.
• Ability to communicate effectively in both English and Spanish preferred.
• Cultural competency.
• Ability to maintain accurate and complete written logs of all activities.
• Access to reliable transportation.
• Ability to work independently.
• Comfortable speaking to groups and meeting new people.
• Able to professionally represent oneself and Imperial Beach Health Center.
