Job Description - Community Partner Liaison

Job Description: The Community Partner Liaison (CPL) will create and maintain trusting and collaborative relationships with community partnerships that serve (0-5 partners, schools, community organizations) and community partner administration and staff. Community Partner Liaisons (CPL) hold monthly meetings with each partnership to discuss referral process, agency updates and “temperature check” meetings to ensure partnerships are operating smoothly. Community Partner Liaison is also tasked with the maintenance and processing of clinical referrals sent to the organization through these partnerships. The Community Partner Liaison serves as the primary point of contact at Valle Del Sol, facilitating connections between patients, families, and necessary services. This role involves processing new referrals from schools and community partners, ensuring patients are promptly scheduled to begin behavioral health services, and performing outreach to community partnerships through activities such as tabling, marketing events, and school events. Job Qualifications: 18 years of age or older; and HS Diploma & 6 months BH experience (direct or program admin) Bachelor’s or Master’s degree in behavioral health (B.H.) related field; or Clinical Intern in Bachelor’s or Master’s degree behavioral health program in good standing Ability to provide culturally sensitive services Strong verbal, written, and communications skills Knowledge of Windows environment and/or Electronic Health Records Systems. Ability to provide culturally sensitive services Must have or be able to obtain a valid Fingerprint Clearance Card. Must possess reliable transportation, valid driver's license, and proof of auto insurance. Must maintain current BLS certification TB Screening Record and PPD/QuantiFERON/Chest x-ray as applicable upon hire Vaccinations/Titer’s Test/Declination Form: Flu, Hep B, MMR, Tdap, Varicella , Covid-19
