Community Relations Manager - In Home Care Agency

We are seeking a dynamic Home Care Consultant who has the heart for senior care. This role is ideal for someone who thrives on building relationships, driving growth, and truly making a difference in the lives of seniors and their families.

  Develop and implement outreach strategies to generate high-quality client referrals

  Build lasting partnerships with healthcare providers, senior centers, discharge planners, and other community organizations

  Represent our agency at community events, health fairs, and networking opportunities to enhance visibility

  
  


• 
  

  Maintain organized, accurate records of referral sources, leads, and follow-ups

  
  


• 
  

  Collaborate with internal teams to ensure a seamless client intake experience

  
  


• 
  

  Consistently achieve or exceed monthly referral and growth targets

  
  

• 
  

  Proven success in community outreach, business development, or sales
  
  (experience in home care, senior services, or healthcare is preferred)

  
  


• 
  

  Excellent interpersonal, communication, and networking skills

  
  


• 
  

  A genuine passion for serving seniors and supporting family caregivers

  
  


• 
  

  Self-motivated, goal-oriented, and able to work independently while collaborating across teams

  
  


• 
  

  Knowledge of home care services, aging issues, or healthcare systems is a plus

  Valid driver’s license, reliable transportation, and comfort traveling within the local service area

  Salary: $45,000–$52,000 annually (based on experience)
