Job Title: RN Case Manager – Flexible Schedule + Growth Opportunity

Location: Greater Twin Cities Metro Area

Schedule: Full-Time (M-F, 8:00 AM – 5:00 PM), 1 weekend/month, 1-2 on-call shifts during the week per month.

Compensation: $85,000 – $130,000 annually + $6,000 in first-year bonuses

What You’ll Do:

• Perform assessments, develop care plans, and conduct skilled nursing visits
  
• Serve as liaison between care providers, clients, and family members
  
• Educate clients and caregivers on chronic disease management and safety
  
• Ensure full compliance with OASIS and state/federal regulations
  

Qualifications:

• Current MN RN license
  
• Minimum 3 years of RN experience
  
• Familiarity with home care documentation and regulations
  
• Valid driver’s license and reliable vehicle
