Job Description - Community Support Facilitator (CSF) (Concord, CA)

Community Support Facilitator (CSF) – Full-Time

At A Bright Future, we provide comprehensive support services to adults with complex developmental disabilities, including those with significant behavioral, medical, psychiatric, and forensic needs. This work demands the highest standards of clinical judgment, ethical leadership, and professional accountability.

We are hiring in Los Angeles and surrounding areas, including Long Beach, Beverly Hills, San Bernardino. This position requires regular travel between program sites across these locations.

We are seeking an experienced Community Support Facilitator who brings proven leadership capacity, independent critical thinking, and the maturity to manage high-stakes situations with confidence and compassion.
This is not an entry-level position. This is a leadership role for someone ready to take ownership of program quality, staff development, compliance, and the safety and dignity of the individuals we serve.

As a Community Support Facilitator, you will serve as a senior member of our leadership team, working directly with the Program Director to ensure clinical excellence, regulatory compliance, and operational stability across multiple program sites.

You will be responsible for:
  • Clinical oversight and independent decision-making in complex, dynamic situations
  • Supervision and professional development of House Managers and Community Living Instructors
  • Crisis intervention and risk management, including de-escalation, emergency response, and real-time problem-solving
  • Regulatory compliance and audit readiness, including documentation quality, ISP/IPP monitoring, and Regional Center coordination
  • Program integrity, including behavior support plan implementation, IHSS oversight, Section 8 coordination, and incident management
This role requires someone who can think critically under pressure, lead with accountability, and maintain professional boundaries while fostering a culture of empathy and excellence.
ESSENTIAL RESPONSIBILITIES
Leadership & Staff Development
  • Supervise, mentor, and evaluate House Managers and Community Living Instructors, ensuring consistency, competence, and accountability across all shifts
  • Lead onboarding, training, and ongoing professional development with a focus on clinical skill-building, ethical practice, and regulatory knowledge
  • Provide real-time coaching, corrective feedback, and performance management as needed
  • Model leadership practices rooted in integrity, emotional regulation, and person-centered care
Clinical Oversight & Service Coordination
  • Coordinate and monitor implementation of Individual Service Plans (ISPs) and Individual Program Plans (IPPs), ensuring fidelity to Regional Center requirements and person-centered goals
  • Collaborate with clinical and behavioral teams to implement behavior intervention plans, monitor progress, and adjust supports as needed
  • Serve as liaison between individuals, families, conservators, Regional Centers, service providers, and internal teams—communicating clearly, professionally, and proactively
  • Oversee health and safety protocols, including medication management, medical appointments, and emergency procedures
Crisis Management & Risk Mitigation
  • Respond to crises with calm, clinical judgment, and adherence to agency protocols
  • Implement de-escalation and Pro-Act techniques during behavioral escalations or emergency situations
  • Assess risk in real time and make independent decisions to protect individuals, staff, and the organization
  • Document incidents thoroughly and follow up with appropriate corrective or preventive action
Compliance, Documentation & Quality Assurance
  • Maintain accurate, timely, and thorough documentation, including progress notes, incident reports, service coordination updates, and compliance records
  • Participate in audits, site reviews, and regulatory inspections with professionalism and preparedness
  • Monitor compliance with state regulations, Regional Center standards, IHSS requirements, and internal policies
  • Identify gaps or risks proactively and collaborate with leadership to address them
On-Call & Operational Support
  • Participate in 24-hour on-call rotation (shared 5-day shifts with Program Director and other CSFs), providing emergency support, guidance, and decision-making authority
  • Travel between program sites regularly (approximately 50% of work time) to ensure consistency and quality across locations
  • Support program operations during staff shortages, transitions, or high-acuity situations

QUALIFICATIONS & REQUIREMENTS
Education & Experience (One of the following required)
  • 2 years of supervisory or leadership experience in developmental disabilities, behavioral health, or related human services
  • OR 5+ years of progressive leadership experience in the developmental disabilities field, including direct supervision of staff and management of complex cases
Mandatory Clearances & Licensure
  • Must be 18 years or older
  • Pass criminal background clearance (DOJ/FBI), drug screening, and TB test
  • Valid California Driver's License with clean DMV record
  • Current automobile insurance and vehicle registration

SCHEDULE & COMPENSATION
Employment Status: Full-Time, Non-Exempt
Schedule: Includes evening, weekend, and on-call rotation coverage
Travel: Ability to travel between program sites regularly (~50% of work time)
Compensation: $23–$25 per hour (Salary negotiable based on qualifications and experience)

WHAT WE OFFER
  • Purpose-driven work in an organization committed to excellence and dignity
  • Leadership growth with mentorship, professional development, and opportunities for advancement
  • A collaborative team that values accountability, learning, and mutual support
  • The opportunity to build something sustainable—to contribute to a program culture rooted in quality, not just compliance

If you’re ready to lead with compassion and help build a brighter future for others. We’d love to meet you.
