Qualifications:
Be 21 years of age.
Have a valid Nebraska Driver’s License and be insurable, have a good driving record and be a confident driver.
If diagnosed with a substance use disorder, individual must have a minimum of 18 months’ recovery/abstinence. Must have the ability to pass an initial and ongoing random drug screenings.
A Bachelor’s degree or higher in psychology, sociology, or a related human service field is preferred. Two years of coursework in a human service field and/or two years of experience/training or two years of lived recovery experience with demonstrated skills and competencies in treatment with individuals with a behavioral health diagnosis is acceptable.
Have a sophisticated set of interpersonal skills, including verbal fluency, interpersonal perception, affective modulation and expressiveness, warmth and acceptance, empathy and focus on others.
Have the ability to display cues of acceptance, understanding and expertise in the first moments of interactions with consumers through verbal and non-verbal behavior.
Possess the ability to form a working alliance with a broad range of clients.
Be willing to become knowledgeable of supportive services and other referral agencies in the community to benefit clients, and develop other community resources.
Have the ability to work with other agency professionals to meet needs and best interest of clients, serving as a liaison between agencies.
Exhibit excellent written and oral communication skills.
Able and willing to practice strict confidentiality guidelines following 42-CFR and HIPAA regulations.
Be resourceful, confident, assertive, have strong, appropriate boundaries, and be able to problem solve in a variety of situations.
Must be able to work effectively as part of a team and independently, and able to discern which skill is needed in a given situation.
Possess good stress management skills.
Exhibit exceptional time management skills.
Be able to follow directives and complete any duties as assigned.
Have the ability to work a flexible schedule and be available on evenings, weekends and holidays (as needed).
Possess a non-judgmental, caring attitude toward the persons served.
Duties and Responsibilities:
Complete required trainings and certifications within established timeframes, and remain current on required certifications.
Maintain a caseload of community support clients.
Complete orientation and intake documentation with all clients within established timeframes
Complete authorizations for services.
Complete an initial needs assessment with each client within 30 days of intake.
Complete an initial service plan with each client.
Update and maintain service plans with each client throughout services.
Meet with each client as needed to provide quality services in accordance with the program they are in.
Complete Community Support contact notes within 24 hours of meeting with each client.
Maintain appropriate releases of information to effectively coordinate services for the client
Monitor client progress and barriers.
Act as the central hub of the client’s treatment team by maintaining ongoing communication with all providers regarding the client’s progress and barriers in their treatment.
Upon the client’s discharge from Community Support, complete a discharge summary within established timeframes.
Provide direct rehabilitation and support services to each client in the community. Examples include;

Providing referrals and coordinating services with other providers and agencies in the community.
Assisting clients in accessing available resources.
Helping clients obtain and maintain safe and affordable housing.
Actively coordinate with Region 4 housing program regarding any clients receiving Region 4 housing assistance.
Providing vocational and educational assistance.
Providing transportation.
Attending court appointments for support and encouragement.
Attending psychiatry and medical appointments to assist the client in advocating, communicating, understanding, and following through with recommendations.
Assisting the client in obtaining medication and maintaining medication compliance.
Coordinating services and providing support for clients in crisis.
Teaching skills including communication, budgeting, coping, relapse prevention, social, independent living, activities of daily living, leisure, decision making, and healthy meal planning.
Wellness Recovery Action Planning.
Providing motivation and support for healthy life changes including diet and exercise.
Building a healthy social support network.

Finding a spiritual community.
Finding a recovery community and/or a sponsor.

Probationary Period:

6 months
