Job Description - Community Support Supervisor

Description

The Community Support Supervisor is a licensed professional who oversees the daily operations and service quality of the Targeted Case Management (TCM) and Care Coordination team. This role is responsible for supervising a team of Community Support Specialists/Case Managers, ensuring compliance with Maryland Medicaid regulations, and, when necessary, providing direct client support or carrying a small caseload. The Supervisor promotes best practices in service delivery, supports the clinical and professional development of staff, and ensures high standards of care. This position is integral to maintaining continuity of care and achieving positive outcomes for clients living with mental health challenges.


Requirements

Essential Job Responsibilities:

  • Supervise up to eight (8) Care Coordinators, providing both clinical and administrative supervision through individual supervision sessions, team meetings, and formal performance evaluations.
  • Ensure all case management services comply with Maryland TCM and Care Coordination regulations, including timely and accurate completion of intake assessments, individualized plans of care, progress notes, and other required documentation.
  • Monitor and manage staff caseloads, service intensity levels, and weekly unit productivity to meet agency benchmarks and funding source requirements.
  • Review and approve documentation for quality, completeness, and Medicaid billing compliance on a regular basis.
  • Oversee onboarding, orientation, and role-specific training for new hires; provide ongoing coaching and professional development for all supervised staff.
  • Collaborate with internal departments (e.g., quality assurance/compliance and billing) to support integrated and coordinated care delivery.
  • Respond promptly to critical incidents, facilitate case reviews, and provide clinical consultation on high-risk or complex client situations.
  • Ensure all services are client-centered, trauma-informed, culturally responsive, and delivered in accordance with clients’ rights and ethical standards.
  • Serve as the primary point of contact for coordination with external partners related to case management services.
  • Track, analyze, and report on key performance indicators (e.g., caseload ratios, service utilization, documentation timeliness, and client outcomes) as required by leadership or funders.
  • Maintain a small caseload or provide direct client support, as needed, to ensure service continuity and meet program needs.
  • Perform other duties as assigned to support program operations and agency goals.

EXPERIENCE AND EDUCATION: 

  • Master’s degree in Social Work, Professional Counseling, Psychology, Nursing, or Medicine
  • Minimum of one (1) year of supervisory experience in behavioral health.
  • Minimum of one (1) year of experience working with children and youth with mental health or co-occurring disorders.
  • Demonstrated ability to provide clinical consultation and training related to mental health or co-occurring disorders.
  • Experience with person-centered planning.
  • Excellent organizational, communication, and coaching skills.
  • Valid driver’s license, access to reliable transportation, and ability to conduct field-based supervision or support as needed. 

Preferred Qualifications:

  • Familiarity with Maryland’s Targeted Case Management (TCM) program
  • Experience using ASO systems (e.g., Optum Maryland) and electronic health record (EHR) platforms 

REQUIRED CERTIFICATION OR LICENSE: 

  • CPR and First Aid Certified 
  • Must be licensed and legally authorized to practice in Maryland under the appropriate Practice Board (LCSW-C, LCPC, Psychologist, RN, MD).
