Description
• Provides trauma assessment and referral as needed.
• Provides in-person case management.
• Develops individualized case plan for each client, including family members as
appropriate to the specific client, including assessment and goal plan.
• Works in a collaborative manner with Senior Program Director and Program Coordinator
in case consultation to ensure that all the needs of each client are assessed and met across
all aspects of the program.
• Provides immediate, short-term, and long-term support to victims and families.
• Builds and cultivates a network of services for clients and facilitates connections to
needed resources, such as housing, legal assistance, mental health services, job
placement, medical, and transportation.
• Connects clients directly with providers through a warm handoff approach and ensures
that services are accessible and provided when needed.
• Monitors community violence notification applications and/or custom notifications from
local law enforcement to stay aware of and respond to incidents of community violence.
• Works on an on-call basis as scheduled 24/7 to respond to violent incidents.
• Performs crisis management at the scene of a violent incident, at the home of the victim
and/or at the hospital.
• Performs victim services to support victim and family in the immediate aftermath of
victimization to stabilize, ensure safety and address emergency needs.
• Coordinates with street outreach workers and other city funded partners, within their
designated community area, to provide immediate crisis intervention, assess retaliation
threat, and implement proactive safety planning.
• Assists families with funeral planning and medical examiner visits.
• Provides support to families during court visits, trials, and other legal processes.
• Performs the role of influential messenger, modeling pro-social and non-violent attitudes and skills to help establish non-violent norms in the community.
• Works with street outreach workers to prevent retaliation.
• Assists families with application to the Illinois Crime Victim Compensation Program operated by the Office of the Illinois Attorney General as well as the Emergency Supplemental Victims Fund (ESVF).
• Assists in administering the ESVF program.
Requirements
• Bachelor’s degree with three years of experience in providing intensive case management and wraparound services to victims of violence or Associate’s degree with four years of experience in providing intensive case management and wraparound services to victims of violence.
• Two years of experience working with highest-risk clients.
• Extensive knowledge of local resources and service providers is preferred.
• Experience with street outreach, victim service and gang intervention programs.
• Training in victim assistance, trauma-informed victim support/counseling, cultural competence, mental health or psychological first aid, and other relevant areas.
