What You’ll Do

• 
  

  Provide individualized care based on the client’s Plan of Care

  
  


• 
  

  Monitor and document changes in the client’s condition

  
  


• 
  

  Communicate proactively with the Director of Nursing and family

  
  


• 
  

  Ensure safe, high-quality in-home care

  
  


• 
  

  Build a meaningful, long-term client relationship

  
  

Requirements

• 
  

  Active, unrestricted RN or LPN license in Indiana

  
  


• 
  

  CPR certification & negative TB test or chest X-ray

  
  


• 
  

  Nursing experience preferred

  
  


• 
  

  Recent graduates welcome with strong clinical skills and reliable availability

  
  


• 
  

  Valid driver’s license and auto insurance

  
  


• 
  

  Ability to lift up to 50 lbs. and assist with mobility as needed
