Contingent Hospice Nurse

University Home Care Inc is seeking a compassionate and dedicated Contingent Hospice Nurse to join our team in Livonia, MI. In this role, you will provide exceptional hospice nursing care, supporting patients and their families during end-of-life care. You will work collaboratively with a multidisciplinary team to ensure personalized, high-quality care that respects the dignity and wishes of each patient.

Responsibilities

• Provide skilled nursing care, including assessment, planning, implementation, and evaluation of hospice patients’ needs.


• Manage symptom control and comfort measures to enhance patient quality of life.


• Educate patients and families about disease processes, prognosis, and care plans.


• Coordinate with interdisciplinary teams to develop comprehensive care plans.


• Document all patient care and communicate changes in patient status promptly.


• Support patient and family psychosocial and spiritual well-being as appropriate.


• Maintain compliance with all regulatory requirements and University Home Care Inc policies.

Requirements

• Registered Nurse (RN) license in the state of Michigan.


• Experience in hospice or palliative care preferred.


• Strong clinical assessment and critical thinking skills.


• Excellent interpersonal and communication skills.


• Ability to work independently and as part of a team.


• Flexibility to work on a contingent basis as needed.
