The nurses of the PCM department provide the integral service of carrying out the initial evaluations and annual re-evaluations critical to allowing the necessary services to begin, continue, evolve etc.

Requirements

Stavros is seeking to hire Contracted Nurses (RN’s/LPN’s) who would be meeting People with Disabilities (aka Consumers) in their homes, located in the Hampden, Hampshire and Franklin counties to complete initial and annual In-Home Evaluations and Re-Evaluations. These evaluations are essential in identifying the available resources and supports needed by the consumers.       Qualified candidates will have the following job requirements: 

• Valid and unrestricted Registered Nurse / Licensed Practical Nurse license. 
• Must be able and willing to travel to complete in-home visitations and evaluations.
• Current CPR certification. 
• Requires at least 1 year of community-based health experience.
• Knowledge of disabilities and community resources. 
• Proficiency with medical documentation and timely submission of documents
• Reliable transportation and proof of auto Insurance is required.
• Bilingual preferred but not necessary (English/Spanish).
