Coordinated Entry & Housing Specialist

The Coordinated Entry & Housing Specialist position entails:

• Coordinates the entry system for homeless clients in Adams, Columbia, Dodge, Juneau and Sauk Counties.
• Assesses and refers clients to housing agencies for placement.
• Enters data which prioritizes clients for appropriate housing service agencies for placement.
• Implements outreach services to inform agencies of the coordinated entry system to target homeless clients most in need of services.
• Recruits agencies to participate in coordinated entry.
• Ensures housing agencies follow coordinated entry standards.

B.S. in Social Work helpful

Preferences considered for experience working with homeless/at-risk clients, conflict resolution skills, and community collaboration.

40 hours per week.
