Rural Coordinated Entry Specialist – SNOFO

Schedule: Monday–Friday, with occasional travel or schedule flexibility
Location: Regional position with travel throughout assigned rural counties


Position Summary


Community Partnership of Southeast Missouri is seeking a dependable, organized, and compassionate Rural Coordinated Entry Specialist to support the Rural SNOFO Coordinated Entry project.


The Coordinated Entry Specialist will help individuals and families experiencing homelessness or housing instability access Coordinated Entry, complete required assessments, connect with appropriate housing and community resources, and remain engaged throughout the referral process.


This position requires strong communication, documentation, time-management, and problem-solving skills. The ideal candidate will be comfortable working independently, traveling throughout rural communities, collaborating with service providers, and working directly with individuals experiencing housing crises.


Essential Job Responsibilities



  • Complete Coordinated Entry screenings, assessments, and required documentation.

  • Assist individuals and families experiencing homelessness or housing instability with accessing housing resources.

  • Enter accurate and timely participant information into HMIS and other approved tracking systems.

  • Maintain contact with participants while housing referrals or services are pending.

  • Update participant contact information, housing status, household composition, and service needs.

  • Participate in case conferencing and housing-prioritization meetings.

  • Coordinate referrals and warm handoffs to housing providers, shelters, outreach programs, and community resources.

  • Build relationships with agencies throughout the assigned rural counties.

  • Provide education to community partners regarding Coordinated Entry and available referral pathways.

  • Assist with identifying individuals who may not otherwise have access to Coordinated Entry.

  • Maintain accurate referral logs, case notes, assessment records, and program documentation.

  • Protect participant confidentiality and follow HMIS privacy and security requirements.

  • Complete required reports and assist with program monitoring.

  • Participate in staff meetings, training, supervision, and community collaboration meetings.

  • Travel throughout the assigned service area as needed.

  • Immediately report safety concerns, suspected abuse or neglect, threats, fraud, or confidentiality concerns.

  • Perform other duties related to Coordinated Entry and the Rural SNOFO project as assigned.


Minimum Qualifications



  • High school diploma or equivalent required.

  • Valid driver’s license, reliable transportation, and acceptable driving record.

  • Ability to travel throughout assigned rural counties.

  • Strong written and verbal communication skills.

  • Ability to maintain accurate and timely documentation.

  • Basic computer skills, including email, Microsoft Office, and web-based data systems.

  • Ability to work independently and manage multiple referrals and deadlines.

  • Ability to work respectfully and professionally with individuals experiencing homelessness, housing instability, trauma, mental-health concerns, substance-use concerns, or other barriers.

  • Ability to maintain confidentiality and professional boundaries.

  • Ability to pass required background screenings.


Preferred Qualifications



  • Associate or bachelor’s degree in social work, human services, psychology, criminal justice, sociology, public administration, or a related field.

  • Previous experience in case management, housing services, homelessness services, outreach, behavioral health, social services, or community-resource coordination.

  • Experience using HMIS or another case-management database.

  • Knowledge of Coordinated Entry, housing programs, community resources, or rural service delivery.

  • Experience working with individuals and families experiencing homelessness.


Relevant professional and lived experience may be considered in place of formal education.

  • Travel and work in community-based settings.

  • Complete documentation accurately and within required timeframes.


Physical and Travel Requirements


This position may require:



  • Regular driving throughout the assigned service area.

  • Entering community agencies, shelters, offices, and other service locations.

  • Sitting, standing, walking, and using a computer for extended periods.

  • Carrying program materials and supplies.

  • Occasional evening meetings, outreach activities, or schedule adjustments.

Compensation


Salary is up to $40,000 annually and will be determined based on relevant education, experience, and qualifications.
