Job Description - Coordinator of Healthcare



Full-time


Description

This position is responsible for managing and coordinating health services for clients enrolled in Agency programs, ensuring the highest standards of care and well-being.

Primary Responsibilities:

Key duties include but are not limited to:

  1. Assessing and reporting ongoing health conditions, ensuring timely treatment and services are obtained, including conducting quarterly physical assessments in line with Agency policies.
  2. Coordinating and promoting optimal health care to support recovery from illnesses or health crises.
  3. Communicating new or additional treatment options recommended by licensed healthcare professionals or specialists promptly.
  4. Participating in team planning meetings, care plan reviews, and committees to provide expert consultation as needed.
  5. Delivering hands-on and onsite training, as well as continuous feedback, to employees responsible for administering medications and treatments.
  6. Educating Agency employees on infection prevention, disease control, nutrition, medical treatments, and mental health topics.
  7. Monitoring medication administration records (MARs) for accuracy and compliance with policies and regulations.
  8. Overseeing inventory and proper storage of medical supplies and medications.
  9. Supporting clients in understanding and managing their health conditions, including assisting with appointments and health-related follow-ups.
  10. Acting as a liaison between the Agency, healthcare providers, and family members to ensure effective communication regarding client health and treatment plans.
  11. Performing regular audits of health documentation and ensuring compliance with local, state, and federal regulations.
  12. Assisting with emergency health situations and providing direct medical care when necessary.
  13. Other duties as assigned

Requirements

Qualifications:

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) degree
  • Active nursing license in good standing
  • Preferred: Minimum of two years of direct patient care experience in a hospital, clinic, or related healthcare setting

Required Knowledge, Skills and Abilities:

  • Strong verbal and written communication skills
  • Familiarity with developmental disabilities and best practices in client care
  • Ability to provide direct medical services under the guidance of primary healthcare providers
  • Skill in developing and delivering staff training programs related to medical care and the management of health issues affecting clients
  • Detail-oriented with strong organizational skills to manage health records, medication inventories, and training schedules.
