Responsible for the coordination and facilitation of State Supported Living Center (SSLC) admissions from initial referral through admission. Duties include gathering and reviewing pertinent records, completing and submitting SSLC admission applications and required documentation, coordinating with SSLCs and other involved agencies, submitting required documentation to the court, attending court hearings related to SSCL admission, tracking admission requirements and timeliness, and maintaining ongoing communication and support with individuals and families throughout the admission process. The position will serve as the primary point of contact for SSLC admission coordination and will ensure timely follow-up, accurate documentation, and continuity of support throughout the admission process. This position will assist in the Determination of Intellectual Disabilities (DID) process by making contacts, coordinating scheduling, and completing any needed follow-up duties. This position may assist with other LIDDA/COS functions and administrative duties to support continuity of services, including data entry, completing waiver enrollments, permanency planning, quarterly reporting, and extension-related activities.
Minimum Qualifications:
Bachelor’s degree or advanced degree from an accredited college or university in any field, with a minimum of one (1) year of paid or unpaid experience working with individuals with intellectual and developmental disabilities (IDD); OR Associate degree from an accredited college or university in a social, behavioral, human services, or health-related field, including psychology, social work, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, criminal justice, or a related field, with a minimum of one (1) year of paid or unpaid experience working with individuals with IDD; OR High school diploma or a certificate recognized by a state as the equivalent of a high school diploma, with a minimum of two (2) years of paid or unpaid experience working with individuals with IDD.
Preferred Qualifications:
Experience with SSLC admissions, IDD/LIDDA services, waiver programs, court processes, or other residential placement systems. Knowledge of HHSC/LIDDA policies and procedures related to SSLC admissions and familiarity with required admission documentation and timelines. Experience coordinating with courts, SSLCs, families, attorneys, providers, and other community partners.
