Key Responsibilities:
- Assist in implementing individualized treatment plans under the supervision of a licensed Occupational Therapist (OT).
- Work with patients to improve their ability to perform daily activities and achieve greater independence.
- Provide direct patient care in the home setting, including therapeutic exercises, functional activities, and adaptive equipment training.
- Monitor patient progress and communicate observations and updates to the supervising OT.
- Educate patients and their families on home exercise programs and safety strategies.
- Ensure accurate and timely documentation of patient progress, treatments, and outcomes.
- Collaborate with the multidisciplinary team, including physicians, nurses, and case managers, to provide comprehensive patient care.
- Adhere to all regulatory and compliance standards in home health therapy services.
Qualifications:
- Current and valid COTA certification.
- Minimum of 1-2 years of experience as a Certified Occupational Therapy Assistant, preferably in home health.
- Knowledge of home health care procedures, patient assessment, and therapy techniques.
- Strong interpersonal and communication skills.
- Ability to work independently with a strong sense of responsibility and initiative.
- Compassionate, patient-centered approach to care.
- Familiarity with electronic health records (EHR) is a plus.
