POSITION PURPOSE
Behavioral health counselor (or clinical counselor or integrated behavioral health clinician). The behavioral health counselor provides direct assessment, intervention, and counseling services to individuals, families, and groups within the primary care setting. The counselor collaborates closely with the primary care team to address the mental health, behavioral, and social needs of patients, promoting overall well-being and integrated care within the RHC.  This job description emphasizes the unique role of a behavioral health counselor in an RHC, focusing on integrated care, collaboration with primary care providers, and addressing the specific needs of rural populations.
 
ESSENTIAL JOB RESPONSIBILITIES
  • Clinical Assessment and Treatment: Conduct thorough assessments of patients to determine their mental and behavioral health needs, including diagnosing psychiatric conditions where appropriate. Develop and implement individualized treatment plans utilizing evidence-based interventions like Cognitive-Behavioral Therapy (CBT), Motivational Interviewing, and brief psychotherapy tailored for the primary care environment. Provide individual, group, or family counseling sessions to help patients manage and overcome emotional and behavioral challenges.
  • Integrated Care and Collaboration: Work closely with primary care providers (PCPs) and other healthcare professionals within the RHC to integrate behavioral health into the patient's overall care plan. This may involve providing warm hand-offs for immediate consultations, participating in team huddles, and offering feedback to PCPs regarding treatment recommendations.
  • Health Promotion and Education: Educate patients about mental and behavioral health conditions, coping strategies, and health behavior changes (e.g., smoking cessation, weight loss, medication adherence). Provide patient education materials and connect patients with community resources when appropriate.
  • Caseload Management and Documentation: Maintain a full caseload and meticulously document patient progress, diagnostic information, and treatment plans in the Electronic Health Record (EHR). Track patient follow-up and clinical outcomes to monitor effectiveness of interventions.
  • Crisis Intervention and Management: Provide crisis intervention and support as needed for patients experiencing acute mental health or behavioral challenges.
  • Referrals and Resource Navigation: Facilitate patient referrals to specialty mental health or substance abuse treatment, social services (e.g., housing assistance), or other community resources when necessary. Assist patients in navigating these transitions and ensure continuity of care.
  • Professional Development: Participate in ongoing continuing education and professional development activities to stay abreast of best practices and maintain licensure.
  • Quality Improvement: Participate in quality assurance activities and initiatives aimed at improving the quality and effectiveness of behavioral health services within the RHC.

QUALIFICATIONS
Minimum Qualifications
  • Education: Master's degree in Social Work, Counseling, Psychology, or a related field, or a doctoral degree in Psychology accredited by the American Psychological Association (APA) or works under the licensure of a licensed professional until hours are met to complete Masters.
  • Licensure: Current, valid state licensure or certification as a Clinical Counselor, Psychologist (PsyD or PhD), or Clinical Social Worker (LCSW) in the state where the RHC operates or working toward the completion of hours of clinical work under the licensure of a licensed practitioner. 
  • Experience: Minimum of two years of clinical experience, preferably in a healthcare or primary care setting, demonstrating proficiency in evidence-based therapeutic techniques.
Skills:
  • Strong understanding of clinical assessment, diagnosis, and treatment planning.
  • Proficiency in evidence-based interventions relevant to a primary care setting, such as CBT and Motivational Interviewing.
  • Excellent communication and interpersonal skills to build rapport with patients and effectively collaborate with a multidisciplinary team.
  • Ability to work in a fast-paced environment and manage brief patient contacts while making accurate clinical assessments.
  • Knowledge of basic physiology, psychopharmacology, and medical terminology.
  • Proficiency with Electronic Health Records (EHRs) and other relevant software and databases.
  • Demonstrated ability to maintain confidentiality and adhere to ethical and professional standards.
  • No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare/Medicaid)
  • Proficient computer skills, including MS Office suite.

Preferred Qualifications
Experience considering the impacts of the work on multiple communities, including communities of color, in technical analysis.
Experience working on a diverse team with different communication styles.
Bi-lingual translation or translation capabilities a plus
 
PHYSICAL DEMANDS & WORK CONDITIONS
  • A typical office environment requires standing, sitting, walking, bending, and lifting up to 25 plus pounds. 
  • Ability to travel and drive to in-person meetings or be on-site as required.
  • May be exposed to patient and environment conditions such as unpleasant sights, smells and contagious diseases (clinic only).
