CRISIS CASE MANAGER

SUMMARY: 

This position works as part of the interdisciplinary team to facilitate and coordinate crisis clients’ continuum of care activities, ensuring optimum utilization of community resources, service delivery, and compliance with external agencies and referral sources requirements. The Crisis Case Manager facilitates and prepares the clients for success in their next phase of treatment and when returning to their community. This position will be cross-trained to assist with other Crisis Services staffing needs such as intake/admissions, mobile crisis, and the crisis center. 

REPORTING:

The Crisis Case Manager will report to the Crisis Intervention Specialist with a dotted line reporting to the CCBHC Case Manager Supervisor.

POSITIONS SUPERVISED:

None DUTIES AND RESPONSIBILITIES:

The Crisis Case Manager’s responsibilities include, but are not limited to:

  • Assess the needs of the clients at admission to determine with the client the goals and the level of assistance required to achieve goals. 
  • Engage with clients throughout treatment to ensure that basic needs are addressed, including required documentation, to allow the client to move forward and achieve goals at discharge. 
  • Educate clients on behavioral health, promote healthy resources, and offer coping skill development techniques to provide the highest quality person-centered care. 
  • Assist clients in developing an aftercare plan so that they will feel confident upon discharge that they have the resources required to facilitate the continuity of care. This includes scheduling aftercare appointments and general case management for clients. 
  • Follow up and complete check-in with all clients discharged from Crisis Services within 24 hours.
  • Maintain case files and other required documentation according to agency policies, including timely submission of documentation by the end of the shift. 
  • Uphold client confidentiality in accordance with company policies and procedures, grant requirements, and local, state, and federal laws and regulations. 
  • Crisis Services clients may exhibit a variety of physical/verbal behaviors and strong emotions. Utilize a trauma-responsive, client-centered, recovery-oriented, and strengths-based approach while maintaining a calm demeanor under pressure. 
  • Work closely with other members of the Crisis Services team to meet client needs, including crisis intervention and mobile crisis as needed. 
  • Liaison with community services, family, friends, partners, and outside resources to create a supportive environment and ensure continuity of care. 
  • Regularly identify and maintain relationships with new employment, housing, medical, and other community resources. 
  • Other duties as assigned. 

MINIMUM QUALIFICATIONS:

Skills/Knowledge/Abilities:

  1. Excellent written and verbal communication skills with the ability to effectively communicate with diverse populations 
  2. Cultural and diversity understanding and awareness 
  3. Interpersonal skills that promote and encourage teamwork 
  4. Advanced knowledge of HIPAA and confidentiality 
  5. Knowledge of behaviors and emotions related to addiction and mental health crises
  6. Advanced knowledge of community and referral resources for clients’ diverse needs 
  7. Skills in documenting effective case management notes, preferably in an electronic health record system
  8. Proficient use of standard office equipment and systems: phone, copier, and computer, including Microsoft Office Word and Outlook tools. 
  9. Critical thinking/problem solving 
  10. Demonstrated ability to prioritize and manage a variety of tasks and organize workload 
  11. Attention to detail 
  12. Maintain effective professional relationships with all levels within the organization 
  13. Familiarity with community resources, including mental health, social, financial, educational, housing, employment, childcare, and other support that may be required. 
  14. Ability to build rapport with clients 
  15. Current CPR/AED/First Aid Certification (or ability to become certified)

Education and Experience:

  1. High School/GED - Required
  2. 1 year of Addiction or Mental Health work experience - Required
  3. CPSW/CFPSW (Certified [Family] Peer Support Worker) Eligibility or Certification – Required
  4. Case Management or Comprehensive Community Support Services (CCSS) Certification (or ability to become certified) - Preferred
  5. Unrestricted NM Driver’s License
  6. Bi/Multi-lingual - Preferred
