Job Description - Day Habilitation Service Manager

Schedule: Monday-Friday 8a-4p

Full time: 40 hours

Salary: $22 per hour

Sign on Bonus: $1,000 for Full time employees, $500 for Part time employees

SUMMARY OF POSITION

The Service Manager reports to the Day Habilitation Program Director and is primarily responsible for the member's case and for the implementation of the member's day habilitation service plan. The Service Manager works closely with the Program Director and clinical team to ensure service delivery and will maintain the member’s record. This position will be compensated on an hourly basis.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Ensure and safeguard Venture consumers’ human dignity, rights and liberties.
  • Demonstrate a high regard for each person and sensitivity for cultural values, expectations and goals of individuals, families, staff and others.
  • Display professional attitude and behavior, being courteous, empathetic, helpful and positive in all interpersonal interactions.
  • Work effectively and cooperatively with individuals, staff, families, community members and others: presenting self and Venture in a positive and professional manner.
  • Focus on providing consumer satisfaction as primary goal: regularly seeking feedback and input from persons supported.
  • Maintain confidentiality by discussing individual concerns only with appropriate people in private setting
  • Actively participate on work teams to continuously improve the services, solve problems, share ideas, and plan for change.

  • Establish and maintain relationships with community based businesses and organizations to develop and enhance valued social roles for consumers.
  • Ensure that services are being provided to consumers in accordance with established DHSP’s.
  • Ensure adequate daily employee coverage and coordination of vacation and other scheduled leave
  • Ensure on-going up keep of service site(s) and vehicle.
  • Ensure adherence to all Venture policies and procedures, and pertinent funding source regulations: including Medicaid, DDS, and CARF.
  • Ensure that Case Record Reviews are auditing ready at all times.
  • Ensure individuals and employee safety and the appropriate handling of emergency situations.
  • Ensure that employees are trained in administering First Aid, CPR, PABC and other emergency care procedures.
  • Conduct monthly emergency drills and complete Venture documentation requirements.
  • Review appropriate policies and procedures annually.
  • Oversee and document supervised employee’ orientation.
  • Utilize experienced employees to enhance training and supervision (mentoring) and oversee employee’s training and development.
  • Monitor supervised employees’ utilization of benefits and allowable absences.
  • Utilize information gathered from Consumer Satisfaction Surveys to develop and maintain quality services.
  • Ensure consumer input guides the DHSP to the greatest degree possible.
  • Responsible for service’s business transactions (i.e. purchase orders, petty cash).
  • Ensure that all personnel information is forwarded to the administrative office in a confidential manner.
  • Develop and submit reports to the Day Hab Program Director quarterly.
  • Actively participate in supervision meetings with the Director of Day Services as scheduled.
  • Inform Day Hab Program Director of issues/concerns as they arise.
  • Establish annual service goals and implementation plans.
  • Establish and maintain positive working relationship with funding source personnel and ensure knowledge of contact requirements.
  • Review and report on a quarterly basis the outcomes of the people served in Habilitation Services and make adjustments as needed to improve services.
  • Integrate the results with DHSP and other areas of service operations.

MARGINAL DUTIES AND RESPONSIBILITIES

  • Maintain confidentiality and positive demeanor while successfully juggling multiple priorities.
  • Attendance at administrative meetings, and other meetings as required.
  • Respond to information requests, as appropriate.
  • Provide recommendations to your supervisor regarding ways to improve the current systems
  • Observe all Agency policies and procedures
  • Accept responsibility and accountability as part of the position
  • Other duties as needed

PHYSICAL AND MENTAL REQUIREMENTS

  • Interact professionally and maintain effective relationships with all other departments within the agency.
  • Actions should reflect judgment that would not jeopardize the health and safety of the individuals.
  • Effective verbal and written communication skills
  • Maintain First Aid, CPR & AED certifications.
  • Maintain trainings specific to individual programmatic issues.
  • Proven ability to meet deadlines and manage multiple priorities as well as a proven ability to lead others.
  • Working knowledge of Microsoft Office and office equipment.
  • Must be able to bend, lift, stoop, and reach to file.

Requirements

  • Bachelor’s degree in relevant human service field preferred.
  • At least three years’ experience and/or training in working with individuals with developmental disabilities and demonstrated working knowledge of principles of normalization.
  • Must be 21+ years old, have a High School Diploma or GED, a valid driver’s license, daily use of a car on site (preferred but not required) and be available by telephone for daily contact.
