As a Support Coordinator, you will assist individuals with intellectual and/or developmental disabilities to coordinate both paid and non-paid services and supports that will aid them in living a fulfilling life within their community. This includes working with the service user and their team to create an individualized service plan that includes the person's aspirations and goals; ongoing monitoring of services and completion of service documentation to ensure that the individual's needs are being addressed and regulatory standards met.

Job Types: Full Time

Compensation: $21.00 per hour
