Job Description - Designated Coordinator

Job Summary and Areas of Responsibility:


The Designated Coordinator areas of responsibilities include: oversight of the facilities programing, nursing, and regulatory requirements. The Designated Coordinator must ensure that the program is in compliance with state and federal regulations as well as facility policies.



Work Hours:


This is a full-time position. Days and hours are Monday-Friday, 8am-4pm with occasional evening and weekends may be required as job duties demand.



Qualifications:


To perform this job successfully, an individual must be able to perform each duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.



  • Must be able to pass an applicant background study

  • 18 years of age or older

  • Possess a valid driver’s license and be insurable

  • Must become certified in First Aid and CPR

  • Must be certified to administer medication within 30 days of employment

  • Must successfully complete VAMR training



Education and/or Experience:


Need to meet DC qualifications below:



  • 4 year degree and 1 year full time direct care with the elderly or developmentally disabled.

  • 2 year degree and 2 years full time direct care with elderly or developmentally disabled.

  • Diploma (accredited postsecondary) and 3 years full time direct care with elderly or developmentally disabled.

  • Minimum of 50 hours of education/training and 4 years of full time direct care under supervision of a staff person who is one of the above.

  • A resume is required to prove direct care experience.



Language Skills:



  • Ability to read and interpret documents such as policies and procedures, medication books and prescription bottles, safety rules and regulations, communication correspondence, meeting documents, etc.  

  • Ability to speak effectively before groups of persons served, administrators, employees of the organization, day program employees, and community members.



Mathematical Skills



  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.



Physical Requirements



  • Must be able to operate a variety of equipment (e.g. wheel chairs, lifts, etc.). Must be able to exert up to 50 pounds of force occasionally and/or frequently to lift, carry, push, pull or otherwise move objects. Physical demands are in excess of sedentary work; position requires walking, standing, reaching, handling, stooping, etc. Will lift no more than 50 pounds independently.



Computer Skills



  • Must have basic computer knowledge and skills. Each person will be trained in computer documentation. All employee trainings are done a secure website which includes competency testing. Employees will record their attendance for work on an electronic device.




Job Duties and Responsibilities:


Other job duties may be assigned as appropriate. The below list is not inclusive and can be changed at any time with or without notice.



  • Make sure the Direct Support Professional's duties are being fulfilled.

  • Training direct support staff.

  • Completing accurate review paperwork and presentation of that information at support team meetings, as needed.

  • Oversight of services and outcome accomplishment.

  • Observation of service delivery and competency.

  • Evaluate and provide feedback with staff job duties, requirements, rules/regulations, and program follow through.

  • Review incident and maltreatment reports and implement corrective actions as needed.

  • Evaluation of satisfaction from the person served.

  • Finding program improvements as needed.

  • Act as a liaison between the person served, facility, medical community, DAC, and other agencies involved in providing services to a person served or the group home.

  • Abide by, enforce, and participate in the implementation and ongoing oversight of all Johnson’s Riverside, Inc. rules and regulations.
