Job Description - Detox / Substance Abuse LVN AM Shift - Urgent Role

Salary: $35-38/hour DOE

Position Summary

The Health Services Coordinator is responsible for providing a variety of community substance use services, which focus on prevention, treatment, education, and rehabilitation of the physical and mental health of all clients participating in West Health’s treatment program. The Health Services Coordinator collaborates with the Medical Director and all program staff as a member of a multi-disciplinary treatment team to develop and implement effective treatment strategies and oversee clients’ health statuses.

 

Responsibilities

  • Responsible for providing a face-to-face assessment of clients within 24 hours of admission to determine the level of care needed, taking medical histories and reviewing the client’s DHCS Client Health Questionnaire (and if appropriate, referring resident to IMS services), document social, psychological, physical and/or behavioral problems related to alcohol and/or other drugs within 24 hours of admission, and ultimately decide if the client is appropriate for IMS services
  • Conduct all activities related to IMS services at the facility, as needed and directed by the Medical Director
  • Review client charts for any new doctor orders, new medications, and related transcription
  • Monitor clients who are going through detoxification by doing rounds, monitoring vitals, and COWS/CIWA, making sure medication orders are being followed
  • Complete inventory for client medications and contact independent affiliated physician as necessary for orders
  • Communicate with independent affiliated physician regarding detox protocol
  • Check in with clients during shift concerning their progress and needs, including the need for any referrals for physical and/or mental health services
  • Identify any issues that may arise which could adversely affect resident health or the quality of care and report them to Program Director and/or Medical Director
  • Consistently document in client charts
  • Review Med Logs to ensure accuracy
  • Monitor client’s self-administration of medication
  • Conduct daily medication counts
  • Send orders out to pharmacy
  • Follow-up with the pharmacy as necessary to ensure medications are delivered promptly
  • Train Behavioral Health Technicians on assistance in the client self-administration of medication
  • Assist staff on duty with admissions, discharges, and transfers during shifts
  • Assist with admitting new clients to facility, including proper, thorough completion of documentation in accordance with policies and procedures, search of items brought to facility, and lock-up of contraband and medications
  • Assist Behavioral Health Technicians in the collection of client UA and breathalyzer results in accordance with policy, as needed
  • Assist Behavioral Health Technicians in conducting and logging rounds
  • Obtain consents from clients; request documentation from previous facilities as appropriate
  • Assess when clients are harmful to self or others and follow the emergency team protocol as necessary
  • Attend and participate in staff meetings
  • Complies with local, state, and federal confidentiality laws and regulations
  • Adhere to DHCS and Joint Commission standards
  • Perform other duties, as assigned

Qualifications

  • Current California LVN or RN license
  • Current CPR/First Aid certification
  • Current TB test
  • Must have at least two years of experience in treating clients with substance use and/or mental health disorders in a residential detoxification setting as well as knowledge of operating within the scope of an Incidental Medical Services license
  • Extensive knowledge of medications and side effects
  • Experience with order transcription, charting, assessments, admissions, and discharges
  • Ability to develop excellent rapport with client, physician, therapist and other program staff
