Salary/Wage: I: $27.89/hr, II: $29.11/hr, III: $31.78/hr

Tribal Level: I: TL 117.1, II: TL 118.1, III: TL 120.1

INTRODUCTION: 

The Diabetes Case Manager will focus on assessing patient needs, developing individualized care plans, coordinating services, advocacy and ensuring appropriate support is received for patients with a diagnosis of diabetes and pre-diabetes. Collaborates closely with Siletz Community Health Clinic providers and tribal partners to enhance access to cultural congruent education and resources to improve diabetes management outcomes and an individual’s overall well-being. This Siletz Community Health Clinic position will be a critical access point for the community to prevent, manage and treat diabetes. Must obtain certification for a Lifestyle Health Coach within 12-months of hire. 

DUTIES:

  • Main facilitator for diabetes education classes for diabetes and pre-diabetes patients.
  • Improve diabetes standards of care through education both group and one on one sessions.
  • Conduct thorough assessments of patient’ needs, strengths, and challenges, including functional, developmental, and mental health needs.
  • Develop individualized case plans that address specific needs and goals, ensuring client choice and participation.
  • Connect patients with appropriate services and resources.
  • Advocate for clients' rights and needs with various agencies and organizations, ensuring they receive the necessary support.
  • Coordinate with other professionals, including providers, and family members, to ensure a holistic approach to care.
  • Regularly monitor patient progress and make adjustments to case plans as needed.
  • Document all interactions and activities, maintaining accurate and confidential records.
  • Evaluate the effectiveness of services and programs, identifying areas for improvement.
  • Build relationships with tribal community members and organizations and assist in representing the tribal diabetes program at cultural sharing events for public education.
  • Collaborate with Tel-Tvm to incorporate the use of traditional foods in the work of diabetes nutrition/education
  • Incorporate traditional practices into food, exercise, medications and lifestyle that is cultural centered.
  • Ability to participate in community events and activities to raise awareness about diabetes prevention and management.
  • Main contact for the Diabetes Prevention Program and Diabetes Education with the Centers for Disease Control (CDC) curriculum.
  • Other duties as assigned.

Requirements

REQUIREMENTS:

TL 117 Diabetes Case Manager I will be required to have an associate degree and one year of health-related coursework or experience working in health, hospital or public in a health or social service-related field –OR- an equivalent combination of education and work and lifestyle experiences with nutrition, positive health practices or experienced as a Lifestyle Health Coach. Must obtain certification for a Lifestyle Health Coach within 12-months of hire. 

TL 118 The Diabetes Case Manager I (DCM I) transitions to Diabetes Case Manager II (DCMII) after obtaining certification for Traditional Healthcare Worker/Community Health Worker. DCM II must meet the DCM I requirements.

TL 120 The Diabetes Case Manager II transitions to Diabetes Case Manager III (DCM III) after obtaining Certified Diabetes Educator, Certified Association of Diabetes Care Education Specialist (ADCES) Diabetes Community Care Coordinator. DCM III must meet the DCM I and DCM II requirements. 

Knowledge of:

  • Electronic health record or electronic medical record system.

Skills to: 

  • Case manages caseloads with a combination of at least 100 diabetes and pre-diabetes patients for resource, education and case managing.
  • Coordinate and deploy health education.
  • Enter patient encounters in an electronic health record or electronic medical record system for billing education services.
  • Collect and submit case management data for grants and patient population health
  • Demonstrate ability to run, monitor and chart results for Hemoglobin A1C machine, Body Composition Scale and retinal camera screener.
  • Demonstrate cultural sensitivity and respect for tribal customs and traditions.

Ability to: 

  • Work independently and as part of a team, strong organizational skills, and commitment to patient-centered care.
  • Follow written curriculum for Diabetes Prevention Program: Coordinate and lead health education curriculum.
  • Follow infection control policies and requirements as well as food safety guidelines, protocols and restrictions.
  • Obtain Life Coach Certification, food handler card and ADCES certifications.
  • Become certified as a Traditional Health Worker.
  • Maintained required certification and continued education in Diabetes Management.
  • Follow written and oral instructions.
