Position Summary

As our Contract Dietitian, you will play a vital role in the Snowline Health team, providing expert nutritional guidance to our patients. Your experience in medical nutrition therapy, patient education, and interdisciplinary collaboration will help us continue delivering high-quality, compassionate care.

Key Responsibilities

• Conduct comprehensive nutrition assessments and develop individualized care plans for patients receiving hospice and palliative care.
• Provide dietary counseling and education to patients, caregivers, and staff to optimize nutritional well-being.
• Collaborate with the interdisciplinary care team to align nutritional plans with overall patient care goals.
• Monitor patient progress and adjust nutrition recommendations as needed.
• Ensure compliance with applicable regulations, policies, and best practices in medical nutrition therapy.
• Document patient interactions and care plans in electronic medical records in a timely      manner.
• Stay current with best practices and evidence-based guidelines in nutrition and hospice care

Requirements

• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
• Current licensure in the state of California.
• Minimum of two years of experience in clinical nutrition, preferably in hospice, palliative care, or home health settings.
• Strong interpersonal and communication skills to effectively engage with patients, caregivers, and healthcare teams.
• Ability to work independently and manage a flexible schedule based on patient needs.
• Knowledge of nutrition-related concerns in end-of-life care, including symptom  management and palliative nutrition strategies.
