DIRECTOR - CASE MANAGEMENT - REGISTERED NURSE / LICENSED CLINICAL SOCIAL WORKER

Progeny HR Solutions LLC is seeking a highly skilled and experienced Director of Case Management to join our team in Atlantis, FL. As the Director of Case Management, you will oversee and lead a team of Registered Nurses and Licensed Clinical Social Workers to ensure the efficient and effective delivery of case management services. Responsibilities: - Develop and implement case management policies, procedures, and protocols to ensure the delivery of high-quality services - Oversee and manage a team of Registered Nurses and Licensed Clinical Social Workers, providing guidance, support, and training as needed - Monitor and evaluate the performance of the case management team to ensure compliance with company standards and regulatory requirements - Collaborate with other departments and healthcare providers to develop and implement care plans for patients - Ensure timely and accurate documentation of all case management activities - Participate in quality improvement initiatives and identify areas for improvement in the case management process - Develop and maintain relationships with community resources to facilitate the coordination of care for patients - Stay updated on industry trends and best practices in case management, and implement changes as needed to improve the delivery of services - Manage the budget for the case management department and ensure cost-effective utilization of resources - Provide support to patients and their families, addressing any concerns or issues related to their care Requirements: - Bachelor's degree in Nursing, Social Work, or a related field - Current and active Registered Nurse or Licensed Clinical Social Worker license in the state of Florida - Minimum of 5 years of experience in case management, with at least 2 years in a leadership or supervisory role - Strong knowledge of case management principles, practices, and regulations - Excellent leadership, communication, and interpersonal skills - Proven ability to manage a team and work collaboratively with other departments - Strong organizational and time-management skills - Ability to work in a fast-paced and dynamic environment - Proficiency in electronic medical records and other relevant software - Bilingual (English/Spanish) is a plus We offer a competitive salary and benefits package, as well as a supportive and collaborative work environment.
