Under the guidance and supervision of the Sr. Director of Government and Community Relations, the Director CDI provides leadership, builds community cooperation, and increases awareness of efforts being implemented in NYS in serving undeserved populations and addressing and ending the HIV epidemic. This position focuses on health disparities, social determinants of health, and the needs of low-income populations; those most vulnerable, those in the Black Diasporic and Latin@ Lesbian, Gay, Bisexual, and Transgender (LGBT) who are victims of domestic and sexual assault; those who are highly impacted by HIV, sexually transmitted infection (STI) and viral hepatitis. Coverage area is the City of Amsterdam, and Albany and Schenectady counties, with an initial focus on the city of Amsterdam and some activities in the City of Albany.
Requirements
Essential Functions:
Provides oversight to the Community Mobilization Program (CMP) in partnership with the AIDS Institute.
Provides oversight to the Outreach and Services to Underserved Program under the Office of Violence Against Women (OVW) grant in partnership with In Our Own Voices. Inc
Designs, plans, and implements community mobilization and engagement activities that increase awareness of HIV and HIV testing, promote sexual health, and build community support among our target populations
Latinx youth, LGBTQ, refugees and immigrants, and intravenous drug users.
Designs, plans, and implements outreach opportunities onsite and within the outlined geographic areas of reach where domestic violence and sexual assault services can be promoted.
Develops work plans, monitors progress and accomplishments in meeting related program goals/objectives.
Responsible for collecting, analyzing, interpreting, and presenting related information/data on local demographics; statistics, trends, and other information that contribute to developing Centro Civico's community needs assessment, CMP program plans, and community engagement activities.
Oversees all program documentation, monitoring and reporting requirements.
Maintains and secures consumer and program data for use in reporting program progress.
Completes and submits required reports, log sheets, and other statistical information that confirms outcomes.
Adheres to all contractual obligations and policies.
Partner with other agencies to develop, implement and evaluate programs for target populations based on data review and collection.
Establish strategic partnerships with other agencies to ensure the effective implementation of these activities.
Serves as an advocate is responsible for developing consumer voice, and facilitating opportunities for increased community lead advocacy efforts, collaborations and healthy relationships.
Provides advanced and culturally relevant training on Sexual Assault, domestic violence and LGBT Inclusion.
Engage in case conferencing on behalf of mutual participants when appropriate.
Organize and participate in prevention events throughout the year.
Develop, with partner agencies, a prevention program for youth and IDU's.
Give presentations on cultural competency in the coverage area as requested.
Develop and implement targeted social media campaigns and written outreach materials.
Network and coordinate programs with other agencies and schools in the community that serve LGBT+, youth, and individuals living with HIV.
Preparation of identified grant proposals
Acquire resources and potential funding opportunities for programs.
Submit timely monthly reports to funders.
Supervise CMP Coordinator and Underserved Program Outreach Worker
Any other duties as may be assigned by supervisor or delegates.
Authority:
This position holds a relatively high degree of independence within the framework of the agency's policies and procedures. The individual must be resourceful and able to display initiative and understanding of management systems and interpersonal relations.
Education & Experience:
Bachelor's Degree in Community Health, Health Science, Political Science, Human Services, Communication, Social Science, or related field preferred
Minimum of three (3) years of professional experience required working in healthcare/community education, public health, human service field or comparable setting, preferably in a not for profit/community-based organization
Direct work experience in community mobilization, health and/or wellness required
Minimum of three (3) years of supervisory/management experience required
Experience working with diverse populations
Bilingual (preferred) in English and Spanish
Knowledge of the grant writing process.
Valid NYS license and access to own transportation
A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO
Physical Demands:
The position does require occasional standing, squatting, lifting of up to approximately 10 lbs. And frequent sitting.
