Role Description:
At Oak Street, Medical Management is focused on supporting high-quality, cost-effective care outside the four walls of primary care clinics. This involves a deep understanding of value-based care, specifically full-risk / capitated arrangements, along with the managed care levers available to drive superior clinical and financial results.
The Director, Medical Management will be instrumental in designing and implementing programs and interventions, targeted to the right patients, to improve quality and cost of care. They will be an effective cross-team leader, working side by side with Treehouse/corporate and field leaders and staff.
Content areas for the Director will include, but not be limited to, transitional care, delegated utilization management & care management, pharmacy and medication adherence, HEDIS / Stars, accurate risk adjustment, and clinical data integration. This will require building relationships with our clinicians, including primary care physicians, nurse practitioners, pharmacists, psychiatrists, registered nurses, licensed clinical social workers, and others.
This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently within Oak Street Health and CVS Healthspireâs Risk Bearing Entity.
This role will report to the SVP, Medical Management, and reports up through the Healthcare Delivery organization at CVS Health. They will have two direct reports and will work closely with leaders across Oak Street Health, the Risk Bearing Entity, and the broader CVS enterprise.
Core Responsibilities:
Serve as strategic operations lead responsible for adapting, designing, and iterating on Oak Street Healthâs medical management infrastructure (i.e., transitional care, delegated services, social care, etc.) to support additional at-risk populations via the Risk Bearing Entity

Manage end-to-end operational and performance reporting, in close collaboration with Data, Product, and Medical Economics teams, to measure and monitor program impact

Own post-acute care program across Oak Street and Risk Bearing Entity, with direct accountability for post-acute costs and utilization, in partnership with medical and operational leaders

Work side by side with Oak Street, RBE, CVS leaders to ensure effective implementation of clinical and business priorities

Manage and support two direct reports to produce high-quality, error-free deliverables

Accountability to key medical management metrics, including quality measures, utilization measures, and financial measures

What are we looking for?
8-10 years of applicable experience in healthcare / population health / working with healthcare providers; or other similar fields

Ability to build the trust and confidence of a wide range of stakeholders

Strong leadership skills and strong influencing skills

Incredible level of organization and detail orientation

Analytical and data savvy. Ability to develop business cases, ROI analysis, and financial analysis

Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives

Expert in MS Office, Google Apps, project management software

Pleasantly persistent approach

Humility

Quick learner comfortable with a high level of ambiguity

Graduate degree preferred but not required (MBA, MPH)

Bachelorâs degree required
