DIRECTOR OF CARE MANAGEMENT

ABOUT THE ROLE 


The Director of Care Management provides leadership, supervision, and operational oversight for Wise & Healthy Aging’s Care Management programs serving older adults and their families. The Director ensures high-quality service delivery, supervises care coordinators, supports staff navigating complex client situations, and maintains compliance with funding, documentation, and program requirements. 


This role combines clinical guidance, program management, and team leadership. The Director works closely with staff supporting older adults experiencing complex needs related to health, housing, safety, and family dynamics. The ideal candidate has strong supervisory experience, knowledge of aging services and community resources, the ability to support staff handling emotionally challenging client situations. Clinical licensure such as LCSW, LMFT, LPCC, RN, or LVN, is preferred but not . 


ROLES & RESPONSIBILITIES 


Program Leadership & Staff Supervision 



  • Supervise and support Care Coordinators providing case management services to older adults. 

  • Guide staff managing complex cases involving safety concerns, family dynamics, or ethical considerations. 

  • Conduct regular case consultations and team meetings to review challenging cases and share best practices. 

  • Provide coaching, mentoring, and professional development for staff.

  • Conduct performance reviews and provide ongoing performance feedback. 

  • Assign caseloads and ensure equitable workload distribution. 


Clinical & Case Management Oversight 



  • Ensure high-quality client service delivery across Care Management programs. 

  • Review client case files to ensure documentation accuracy, timeliness, and compliance. 

  • Support staff in developing effective care plans addressing medical, mental health, housing, and social service needs. 

  • Provide direct client support in particularly complex or high-risk cases when needed. 

  • Provide clinical supervision for Associate Social Workers (ASWs) working toward licensure (if licensed). 


Program Operations & Compliance 



  • Ensure compliance with all program contracts, grants, and regulatory requirements. 

  • Monitor documentation, billing units, and service delivery to ensure accuracy and compliance. 

  • Oversee client data management systems and ensure records are complete and accurate. 

  • Prepare for audits and ensure proper documentation standards are maintained. 


Organizational Collaboration 



  • Serve as a liaison with city, county, and partner agencies regarding client matters. 

  • Participate in community meetings related to elder services and client advocacy. 

  • Collaborate with other Wise & Healthy Aging programs supporting older adults and caregivers. 


Training & Program Development 



  • Provide onboarding and ongoing training for Care Coordinators. 

  • Help refine assessment protocols and care management practices based on best practices. 

  • Supervise MSW interns and student placements. 


QUALIFICATIONS 


Education 



  • Minimum education requirement: A master’s degree in social work, psychology, public health, or a related field. 


Licensure 



  • LCSW strongly preferred  

  • Other clinical licenses such as LMFT, LPCC, RN, or LVN will be considered. 


Experience 



  • Minimum 5 years of experience in social services, case management, or aging services. 

  • 2+ years of supervisory or management experience. 

  • Experience supporting staff handling complex client situations. 

  • Knowledge of community resources for older adults and caregivers. 

  • Experience working with older adults or aging services. 

  • Experience working with diverse and underserved populations. 


Preferred Experience 



  • Experience in nonprofit or community-based organizations. 

  • Experience supervising staff conducting field-based work. 

  • Bilingual language skills are a plus. 


Other Requirements 



  • Ability to measure and assess the impact of services and support and participate in critical elements of the program's success. 

  • Strong clinical, interpersonal, verbal, and written communication skills. 

  • Ability to effectively communicate expectations to team members and demonstrate team building. 

  • Ability to multitask effectively and prioritize work efficiently with competing deadlines. 

  • Ability to demonstrate sound judgment and decision-making skills. 

  • Proficient knowledge of Microsoft Office software programs. 

  • Valid driver’s license, auto insurance, and satisfactory criminal background check required. 


COMPENSATION & BENEFITS 


The salary range for the Director of Care Management is $110,000 - $120,000. Wise & Healthy Aging also offers competitive benefits. For staff who regularly work 30 or more hours per week, benefits include up to 80% company-paid medical, 100% company-paid dental, 401(k) plan with employer match, Flexible Spending Account (FSA), optional life and vision insurance, 14 paid holidays plus paid time off (PTO).
