Director of Case Management

A healthcare facility west of Rochester, NH is seeking a compassionate Director of Case Management to join their team! Details are listed below.

Pay Range: $105,000 - $120,000 / Annually

*Pay based on experience and qualifications

Director of Case Management Opportunity:

• Full-time, direct-hire opportunity
  


• Monday - Friday, Day Shift
  


• Occasional On-Call required
  


• Possibility of quarterly bonuses
  

Director of Case Management Responsibilities:

• Director of Case Management assumes responsibility for the day-to-day operations and human resource management of the Case Management department
  


• The Director of Case Management has 5 Direct Reports underneath them
  


• Provides guidance and support to Case Managers and other staff, including training on managing caseloads and interpreting regulations, policies, operational procedures, and objectives
  

Director of Case Management Qualifications:

• Must be either a Licensed Registered Nurse, Licensed Social Worker, or Therapist
  


• 3+ years of management experience
  


• 3+ years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
  


• CCM® or ACMTM certification is preferred

• A 34-mile trail suitable for walking, biking, and cross-country skiing, close to Lake Sunapee
  


• The city hosts the largest antique shop in New Hampshire, featuring over 155 dealers with a wide range of items
  


• A public park with walking trails, a pond for ice skating in winter, and a large playground
