Job Description - Director of Case Management

A facility near Yuma, AZ is seeking a Director of Case Management to join their team! Comprehensive Benefits Program including generous PTO and benefits starting day 1

Director of Case Management Opportunity:

Full-time, permanent opportunity

Schedule: Days; Mon-Fri

Oversees case management operations, including care coordination, discharge planning, and utilization review

Leads and develops case management staff to support quality outcomes, regulatory compliance, and patient satisfaction

Collaborates with physicians, nursing leadership, and interdisciplinary teams to optimize patient throughput and resource utilization

Director of Case Management Qualifications:

The Director of Case Management must have an LCSW or RN license

Must have 3 or more years of hospital based case management experience

Have or be willing to obtain CCM or ACM

Bachelor's Degree or higher required
