Director of Clinical Strategy and Innovation

We partner with health plans, providers, and other risk-bearing entities to improve outcomes for their highest acuity patients by deploying a value-based and tech-forward care navigation service. As our Director of Clinical Strategy & Innovation, you won’t just oversee operations, you’ll shape the delivery of care across all clinical programs including palliative care, Enhanced Care Management (ECM), advanced care navigation, and hospice care.

This role is designed for a strategic and visionary clinical leader who can scale high-impact care models across diverse populations and care settings. If you thrive at the intersection of innovation, systems-building, and purpose-driven leadership, we want to hear from you.

What You’ll Do

Strategic Clinical Leadership Across All Programs

• Lead and align clinical delivery across palliative, ECM, care navigation, and hospice services.

• Develop and execute unified care delivery models that integrate clinical best practices with scalable, tech-enabled workflows.

• Build forward-looking clinical strategies that drive quality, efficiency, and impact across all service lines.

Team Leadership & Culture Building

• Recruit, coach, and retain a high-performing interdisciplinary clinical team across multiple lines of service.

• Foster a mission aligned culture that inspires the team to love working here

• Roll out dynamic training and upskilling programs

Quality, Compliance & Risk Management

• Ensure compliance with all regulatory requirements, including CMS, Title 22, DHCS, and ACHC accreditation standards.

• Identify and mitigate clinical risks across populations with complex and high-acuity needs alongside our compliance consultant.

Data-Driven, Tech-Enabled Care

• Partner with product and operations teams to ensure proper use by the clinical team.

Who You Are

• A licensed RN in California (required), with 10+ years of case management experience

• 5+ years of leadership experience in hospice, palliative, or population health/complex care.

• A cross-functional collaborator who translates between clinical, operational, and product teams with ease.

• A builder and visionary who thrives in fast-paced, mission-driven environments and values inclusivity, impact, and innovation.

Bonus if You Have

• Experience launching or scaling Enhanced Care Management (ECM) or complex care navigation programs in California.

• Familiarity with CalAIM initiatives and whole-person care models.

• Experience with value-based care programs or alternative payment models.
