The Staff Pad is proud to partner with a reputable,
innovative primary care practice focused on delivering exceptional care to
older adults across office, home, and senior living settings.

We are seeking an experienced, board-certified
primary care physician with strong geriatric experience to join the
multidisciplinary team as Director of Community Geriatrics in Boston, MA,
providing clinical leadership, direct patient care, and oversight of advanced
practice providers and senior living programs.

Schedule: Full-Time | On-call coverage as scheduled

What You’ll Do

Reporting to the President and Chief Medical
Officer, you will be responsible for:

Clinical Care

  • Deliver comprehensive care to older
    adults across outpatient, home, and virtual settings

  • Manage acute and chronic conditions
    alongside NPs and multidisciplinary care teams

  • Lead annual wellness visits, preventive
    screenings, and geriatric assessments

  • Participate in scheduled on-call
    coverage

Facility Oversight

  • Provide medical oversight for patients
    residing in SNFs, ALFs, and LTC facilities

  • Conduct regular rounds to evaluate
    clinical outcomes, quality measures, and care coordination

  • Serve as Medical Director for affiliated
    senior living facilities, supporting quality assurance, policy development, and
    staff education

Supervision and Leadership

  • Lead and mentor NPs and APPs through regular
    chart reviews, joint patient rounds, and ongoing support

  • Partner with interdisciplinary teams to
    coordinate seamless transitions of care, strengthen communication across
    facilities and providers, and reduce hospital re-admissions

Quality and Program Development

  • Lead or participate in quality
    improvement initiatives that enhance care for older adults

  • Develop and monitor performance metrics
    for preventive care, chronic disease management, and patient experience

  • Support ACO population health
    initiatives through proactive gap closure and patient outreach

Risk Adjustment and Compliance

  • Ensure accurate and HCC and ICD-10 risk
    adjustment coding across clinical documentation

  • Partner with compliance and data teams
    to monitor coding accuracy and drive improvements

What We Are Looking For

Qualifications & Experience

  • MD or DO degree from an accredited
    medical school.

  • Board certification or eligibility
    in Internal Medicine or Family Medicine; subspecialty certification and/or
    extensive experience in Geriatrics strongly preferred

  • Active medical licenses in both
    Massachusetts and New Hampshire (required)

  • DEA license and controlled
    substance registration in Massachusetts and New Hampshire

  • 3+ years of experience in
    geriatric, long-term care, or complex primary care preferred

  • Experience supervising advanced practice
    providers (APPs) strongly preferred

  • Experience with value-based care
    programs, including Medicare’s Advanced Primary Care Management Program
