Job Description - Director of Community Support

Description

Greater Nashua Mental Health is looking for a Director of Community Support Services to join our Team! The Director oversees the day to day clinical operations and services with the CSS Department, providing leadership and guidance to CSS Clinical Team Managers and Coordinators. Ensures the effective coordination of quality patient care services. Collaborates internally across departments to develop effective strategies and workflows that support client needs and the agencies goals and objectives.  Provides management and guidance to the department based on: agency policy and procedures; state and federal regulations; contractual requirements; and outcome data on key performance metrics and the overall functioning of the program. Provides specific recommendations and suggestions regarding the department in order to assure quality consumer care.

Duties & Responsibilities: 

  • Supervises staff, ensuring that work is in accordance with the Agency's professional standards, the Program's Plan for Services, and the Agency's Policies and Procedures 
  • Monitors that clients are receiving the appropriate level of care within the department based upon their clinical presentation and functioning needs 
  • Ensures adequate coverage for services and staff supervision during CSS operating hours 
  • Communicates, collaborates, and consults with clients, staff, and other agencies to ensure effective and efficient operations of the program
  • Ensures that established treatment plans are carried out and that clients are treated in accordance with Agency client rights policies and procedures. Ensures quality and appropriateness of critical aspects of care through ongoing monitoring of the established individuals and the appropriate development and implementation of corrective action as indicated
  • Monitors the distribution of caseload based on the assessed needs of clients, as well as existing caseloads and expertise of staff members 
  • Supervises the screening of potential program staff and makes recommendations as to hiring so that departmental objectives can be met
  • Serves as a liaison between CSS and other staff members and departments serving the population's needs, informing them of Agency plans, changes, goals, and policies and procedures
  • Represents the program outside the Agency, serving as a representative on various committees in the community and other mental health agencies within the State, as appropriate to the benefit of the program and the population served
  • Ensures continued educational needs of staff are met within the limits of Agency resources 
  • Maintains client records in accordance with GNMH standards
  • Provides treatment/counseling to adults and families based on the established treatment plan using the approved modalities of the Agency 
  • Develops and updates program plans, ensures monitoring of implementation, and establishes and executes corrective actions as indicated in conjunction with Management and Professional Staff 
  • Ensures regional planning is executed  
  • Develops and oversees the department budget 
  • Completes other required administrative, statistical, scheduling, supervision, data processing, quality assurance, financial management, safety, and building related tasks as assigned
  • Adheres to the agency’s Core Values and Code of Ethics policy, maintaining the highest standards of professional conduct and integrity
  • Performs other duties or special projects as required or assigned  

Requirements

Qualifications: 

  • Master’s Degree in Clinical/Administrative or similar field 
  • Licensed NH Clinician
  • 6 to 8 years related work experience
  • Leadership ability in work setting. Flexible, willing to learn, and anticipates the needs of self and staff
  • Knowledge of Microsoft Office products (Email, Word, EXCEL)
  • Strong collaborating skills, and an ability to work effectively both independently and as part of a team
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive
  • Excellent communication and interpersonal skills
  • Ability to multitask, and be flexible with regard to workload and assignments
  • Ability to exercise judgment  
  • Ability to problem solve and exercise judgment
