Responsible for overseeing and coordinating the process of transitioning patients from healthcare facilities to appropriate lower level, other hospital settings and lateral transfers. This role involves developing and implementing efficient discharge plans, collaborating with healthcare teams and placement coordinators to identify suitable placements as well as finding suitable placements for patients who are in need of placement, ensuring compliance with regulations, and optimizing patient outcomes through effective post discharge support and placement.
Key Responsibilities:
- Develop and implement strategic plans for discharge planning and patient placement, ensuring alignment with the organization’s goals and objectives.
- Evaluate and enhance existing discharge processes to improve efficiency and patient outcomes.
- Collaborate with healthcare professionals including physicians, nurses, social workers and case managers to coordinate and execute effective discharge plans.
- Foster strong relationships with external healthcare partners, community resources, acute care facilities and post-acute care facilities.
- Collaborate with placement coordinators to find appropriate placements for patients with no placement or in need of more appropriate placement.
- Stay informed about relevant healthcare regulations and ensure compliance with discharge planning guidelines.
- Implement policies and procedures to meet regulatory standards and improve patient care quality.
- Work independently, directly with residents and other providers of all skills and levels in the community in order to safely discharge residents to safety with all interventions set up for the home for the ability for the patient to thrive independently.
- Conduct thorough assessments of patients’ needs and potential challenges related to discharge.
- Develop individualized discharge plans that address medical, social and psychological aspects, considering patient preferences and available resources.
- Educate patients and their families on post discharge plans, medications, and necessary follow up appointments.
- Educate patients and their families on appropriate placement options and resources.
- Provide resources and support to empower patients in managing their health after leaving the healthcare facility.
- Monitor and evaluate the effectiveness of discharge plans through data analysis and feedback mechanisms.
- Implement continuous quality improvement initiatives to enhance the overall discharge process.
- Ensure clear and effective communication among healthcare team members and with patients and family members.
- Maintain accurate and detailed documentation of discharge plans, interventions and outcomes.
Education:
High School Diploma or GED; bachelor’s degree preferred.
Skills/Knowledge/Experience:
· 4 years as a Social Services Designee in a SNF or at least 2 years as Social Services Director in a SNF
· Excellent organizational skills
· Excellent verbal and written communication skills
· Demonstrated problem solving skills
· Computer and common computer program literacy (MS Word, Excel, PowerPoint)
· Experience with the patient discharge process and working with external healthcare partners, including knowledge of what community resources are available to patients and how to access them.
-Ability to forge good relationships with hospital contacts, physicians, patients, patient’s families, home health's, DME companies, hospice personnel, case management, Board and cares, assisted livings, business developers, intake coordinators and other discharge planners
