Job Description - Director of Health & Wellness - LPN/RN

Description

 

Clinical Oversight & Resident Care

  • Coordinate and oversee resident health services within the scope of LPN licensure.
  • Conduct resident assessments, reassessments, and care plan updates as required by DHSS and DMH.
  • Communicate changes in resident condition promptly with physicians, families, and interdisciplinary team members.
  • Oversee medication administration systems and ensure compliance with state regulations and community policy by completing routine audits.
  • Provide hands-on nursing care when necessary to support resident needs.
  • On-Call for resident changes in condition, communication and direction for clinical needs. 

Compliance & Regulatory

  • Ensure compliance with all applicable federal, state, and local regulations governing assisted living / residential care. 
  • Maintain accurate and complete clinical documentation in the electronic health record. 
  • Participate in surveys, inspections, audits, and follow-up corrective action plans as required. 
  • Monitor infection prevention and control practices. 
  • Ensure incident reports, investigations, and follow-up actions are completed timely and accurately.
  • Supervise, mentor, and support nursing and care staff.
  • Implement class education for Level 1 Medication Aides and ongoing updates needed.  
  • Provide ongoing education and coaching related to resident care, documentation, inserves, competency evalutions and best practices. 
  • Promote teamwork, professionalism, and effective communication across departments. 

Care Coordination & Communication

  • Serve as a liaison between residents, families, physicians, pharmacies, home health, hospice, and other healthcare providers. 
  • Participate in care conferences and family meetings as appropriate. 
  • Collaborate and manage admissions of assigned communities, coordinate with teams to support safe and appropriate move-ins and transitions of care. 

Administrative & Operational Support

  • Assist the RCF/ALF Managers with health-related operational needs. 
  • Support emergency preparedness, drills, and response procedures. 
  • Contribute to quality improvement initiatives and resident satisfaction efforts.
  • Tracking and completion of DHSS Community Based Assessments in the ALF communities.
  • Tracking and completion of DHSS and DMH related compliance with educational needs of staff, clinical documentation of residents records. 

Requirements

  • Valid LPN or RN license in the state of Missouri is required. 
  • Missouri L1MA Educator Certification.
  • Community Based Assessment Certification is required, if not ability to acquire within 60 days of hire.
  • Home and Community Based Services Reassessor Certification, if not ability to acquire within 60 days of hire.
  • Ability to obtain RCAL (Residential Care Assisted Living License)
  • Minimum 1 year of experience in a healthcare setting, preferably in home health, assisted living or long-term care is preferred. 
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Experience with electronic health records (EHR).
  • Experience and strong knowledge working with Microsoft software, TEAMS, and OneDrive.
