Position Summary


The Director of Nursing (DON) is responsible for the overall clinical leadership, quality, and nursing operations of the Home Health Agency. The DON provides direct supervision of all field clinicians and is accountable for clinical practice standards, staff development, patient care quality, regulatory compliance, survey readiness, and clinical outcomes.


The DON serves as the primary clinical leader of the agency and ensures that patient care is delivered safely, effectively, and in accordance with all federal, state, payer, and accreditation requirements. The Manager participates in development and implementation of the agency’s policies and procedures, confers with appropriate administrative staff on interdisciplinary problems/issues.


The DON embraces the Graceworks Mission, Values, and commitment to Faithful Caring. When the DON is unavailable, an individual is assigned to act in his or her place. The DON assumes 24 hours/day responsibility and accountability for delivery of quality client care, adherence to approved budget, and personnel management. The DON functions as a staff resource and a role model which influences attitudes, modifies behaviors and provides staff development. This position reports directly to the Administrator and is responsible for keeping the Administrator informed of the agency activities, personnel issues and client/family conditions.


 


Position Qualifications:


~Nursing degree required with a specialization in nursing administration, health services, gerontology, and/or business administration. Current Ohio registration as a professional nurse; or application for such licensure by endorsement and possession of letter from state authorizing practice as a registered nurse.


~Valid drivers’ license with safe driving record.


Minimum Experience: Evidence of continuing education. Minimum of five (5) years experience in supervision and management in home care and a home health agency. Communicates clearly in English, both oral and written.


Per CMS: Training and experience in health services administration and at least one year supervisory or administrative experience in Home Health Care or related health programs.


Position Accountabilities and Performance Criteria:


A. Clinical Leadership & Patient Care Oversight



  • Provide overall clinical leadership for the Home Health Agency

  • Ensure safe, appropriate, and effective patient care delivery

  • Serve as the final clinical authority for patient care issues

  • Review and resolve complex clinical situations

  • Provide consultation and guidance to field clinicians

  • Participate in case conferences and interdisciplinary care planning

  • Review high-risk, complex, and unusual patient cases



B. Field Clinician Supervision & Performance Management



  • Supervise all field clinicians

  • Conduct employee evaluations

  • Monitor clinician performance and productivity

  • Provide coaching, mentoring, and professional development

  • Address performance concerns and implement corrective action when necessary

  • Maintain documentation related to performance management

  • Support staff retention and engagement initiatives



C. Staffing & Workforce Management



  • Review and approve clinician PTO requests

  • Monitor staffing capacity and workload distribution

  • Coordinate coverage for absences and staffing shortages

  • Participate in recruitment, interviewing, hiring, and onboarding

  • Make staffing recommendations based on census and operational needs



D. Orientation, Training & Competency Oversight



  • Oversee clinician orientation programs

  • Ensure completion of tablet and technology training

  • Verify clinician competency before independent patient care

  • Oversee annual skills validations and competency assessments

  • Identify educational needs and coordinate training initiatives

  • Maintain compliance with required licensure and competency standards



E. Quality, Compliance & Clinical Documentation Oversight



  • Monitor clinical documentation quality and timeliness

  • Review OASIS accuracy and outcome trends

  • Monitor quality indicators and clinical performance metrics

  • Review hospitalization, readmission, and adverse event trends

  • Collaborate with the QA Coordinator on quality improvement initiatives

  • Collaborate with the Compliance & Transitions Coordinator regarding clinical documentation concerns

  • Ensure clinical compliance with regulatory and accreditation requirements



F. Regulatory & Accreditation Readiness



  • Maintain readiness for Joint Commission, state, and payer surveys

  • Lead preparation for audits and surveys

  • Respond to clinical audit findings and corrective action plans

  • Ensure implementation of clinical policies and procedures

  • Monitor adherence to agency standards of practice



G. Clinical Reporting & Operational Oversight



  • Review clinical reports and dashboards regularly

  • Monitor:


    • Documentation timeliness

    • Productivity metrics

    • Visit utilization

    • Quality outcomes

    • OASIS measures

    • Hospitalization rates


  • Identify trends and implement improvement strategies



H. Policy & Procedure Management



  • Develop, implement, and maintain clinical policies and procedures

  • Ensure policies remain compliant with current regulations

  • Educate staff regarding policy updates

  • Provide interpretation of clinical policies and standards



I. Leadership Team Participation



  • Participate in agency leadership meetings

  • Collaborate with the Administrator regarding strategic planning

  • Assist with agency growth initiatives

  • Support development of operational and clinical goals

  • Provide recommendations regarding clinical operations and staffing



Required Qualifications



  • Registered Nurse license in good standing

  • Home Health experience required

  • Supervisory or management experience preferred

  • Strong knowledge of Medicare Conditions of Participation

  • Knowledge of Joint Commission standards

  • Experience with OASIS and clinical quality measures

  • Strong leadership, communication, and organizational skills

  • Ability to lead interdisciplinary teams
