Position Summary

The Director of Nursing (DON) is responsible for the overall direction, coordination, and evaluation of clinical services in compliance with Medicare Conditions of Participation, state regulations, and agency policies. This role provides leadership, oversight, and support to skilled nursing staff while maintaining high standards of patient care and documentation.

Key Responsibilities

  Provide clinical leadership and oversight of all skilled nursing services

  Ensure compliance with CMS Conditions of Participation, state regulations, and accreditation standards (CHAP)

  
  


• 
  

  Oversee OASIS accuracy, documentation timeliness, and clinical quality outcomes

  
  


• 
  

  Supervise, train, and support skilled nursing staff (RNs/LPNs)

  
  


• 
  

  Participate in hiring, orientation, competency validation, and performance evaluations

  
  


• 
  

  Review and approve plans of care, physician orders, and clinical documentation

  
  


• 
  

  Lead quality improvement initiatives (QAPI) and corrective action plans

  
  


• 
  

  Collaborate with Administrator, physicians, therapists, and interdisciplinary team members

  
  


• 
  

  Participate in survey preparation, audits, and regulatory visits

  
  


• 
  

  Ensure patient safety, infection control, and professional standards of care

  
  

Qualifications

• 
  

  Active Registered Nurse (RN) license in the state of Virginia

  
  


• 
  

  Minimum 2–3 years of home health skilled nursing experience required

  
  


• 
  

  Prior DON or Clinical Manager experience strongly preferred

  
  


• 
  

  Strong working knowledge of Medicare regulations and OASIS

  
  


• 
  

  Proven leadership, organizational, and communication skills

  
  


• 
  

  Experience with survey readiness, audits, and corrective action plans

  
  


• 
  

  Current CPR certification

  
  


• 
  

  Ability to lead calmly, decisively, and professionally
