Job Responsibilities:

Providing oversight and support to Case Managers

Monitoring pediatric and adult patient care

Managing referrals and assigning clinicians

Coordinating care

within the organization and with outside agencies

Evaluating patient outcome scores related to value-based purchasing.

Continuously assessing patient needs and using a team approach to assist patient and family in meeting those needs

Developing, implementing, and updating plans of care

Qualities:

Proficiency

in technology, with the ability to navigate multiple platforms

Effective communication skills in both in-person and virtual settings

Strong organizational and delegation abilities

A team-oriented mindset with a focus on growth

Requirements:

A valid Colorado RN license in good standing

Minimum of two years of management experience in home health,

including

OASIS experience
