Position Summary:
The Director of Outpatient Therapy & Assisted Living Partnerships will lead the launch, development, and growth of Patriot at Home’s outpatient therapy division while driving strategic partnerships with assisted and independent living communities across Ohio. This dual-role leader will ensure operational excellence, regulatory compliance, and financial sustainability of the division while also managing business development efforts to expand market reach and revenue. Working closely with internal teams and external stakeholders, this individual will be responsible for developing scalable outpatient therapy operations, building a strong menu of services, negotiating service agreements, and aligning clinical services with community needs.
The responsibilities of our Director of Outpatient Therapy & Assisted Living Partnerships include:
Licensing & Compliance
Lead the process to obtain and maintain necessary licensing for outpatient therapy services.
Ensure compliance with all applicable state and federal regulations, policies, and standards.
Strategic Operations & Program Development
Create and standardize scalable processes and workflows to ensure efficiency and sustainability.
Oversee implementation and delivery of outpatient therapy programs, working with Part A, Hospice, and Part B services.
Develop and manage the division’s operational budget and performance metrics (KPIs).
Sales & Business Development
Identify and pursue partnership opportunities with assisted and independent living communities.
Promote outpatient therapy and home care services to community administrators and leadership.
Conduct presentations, meetings, and follow-ups to secure contracts and long-term relationships.
Service Menu & Marketing
Design and maintain a robust menu of services including: - Home Health Part A therapy - Outpatient Part B therapy - Disease-specific programs (e.g., CHF, COPD, Dementia) - Wellness and facility engagement events
Collaborate with internal marketing and clinical teams to communicate value to partners.
Negotiation & Relationship Management
Negotiate service contracts and partnership agreements.
Serve as the primary liaison with partnered facilities, ensuring satisfaction and ongoing engagement.
Provide insights and feedback from partners to guide continuous program improvement.
Team Leadership
Recruit, onboard, and manage a team to support outpatient therapy services across locations.
Foster a culture of accountability, excellence, and collaboration.
How This Role Contributes to the Team:
This hybrid leadership role integrates operational and sales expertise to drive the successful launch and long-term sustainability of Patriot at Home’s outpatient therapy division. By aligning clinical offerings with the needs of assisted living communities, the role supports the organization’s mission of providing comprehensive, integrated care throughout Ohio.
Qualifications:
5+ years in healthcare management, business development, or outpatient therapy leadership preferred.
Must hold clinical license in either Speech Language Pathology, Physical Therapy, Occupational Therapy, Physical Therapy Assistant, or Certified Occupational Therapy Assistant.
Proven track record in operational development, relationship building, and contract negotiation.
Strong communication, organizational, and problem-solving skills.
Willingness and ability to travel frequently throughout Ohio.
Job Conditions:
This position involves regular travel across Ohio.
See patients as needed in the field.
Primarily field-based with frequent in-person meetings and facility visits while maintaining a presence in the office when needed.
Requires excellent verbal and written English communication skills.
Must be able to sit, stand, walk, and use a telephone and PC for extended periods.
