The Columbia Presbyterian Community is seeking a highly skilled and compassionate Director of Social Services to join our team to create a warm hospitality experience for all our residents, staff and visitors. In this position, you will enjoy working with a passionate team to provide quality service throughout our community.

What You'll Do:

• Facilitate trust, initiating assessment of resident and family psychosocial status, coordinating and clarifying patient-specific financial aspects of care, and collaborating positive linkages with referral sources.
• Admit residents following the appropriate policies and procedures outlined by state, federal and organizational regulations and guidelines.
• Establish and maintain waiting list, ensuring that applicable state, federal and organizational regulations and guidelines are followed.
• Assist residents with their adjustment to their new living conditions and community.
• Serve as a first point of contact related to concerns of residents and/or family members; initiate resolutions.
• Conduct SNF/Rehab prospect tours and visitor tours and provide comprehensive information about the community.
• Encourage family contact and communicate with families on an as needed basis; assist residents with various forms of communication to family.

What We Are Looking For:

• Associate’s degree in Social Services or related field.
• 1 year of long-term care experience.
• 1 year of Medicare admission experience.
• Valid driver’s license issued by the State of South Carolina
