Job Description - Discharge Planner


Title: Discharge Planner for Inpatient Behavioral Health

Salary: $64,000-$74,000

Location: Cushing, OK (onsite)

Job purpose

  • This position is responsible for supporting insurance verification, pre-certification, and certification, as well as navigating the necessary systems for patients requiring placement and discharge. This position also collaborates with the community to ensure the appropriate discharge planning of all patients.

As PMC’s Discharge Planner, your focus will be on the 3Cs; Care, Community and Compliance.

 

Care – Provide the best possible patient care

  • Educates patients and their families on the signs and symptoms of illness to prevent rehospitalization and improve disease management skills.
  • Provides assessments, treatments, and case management to patients and families.
  • Participates in discharge planning, arranges for follow-up care, and develops mechanisms for the exchange of appropriate information with sources outside the hospital.

Compliance – Ensure the program operates within all regulations

  • Maintains and participates in performance improvement activities within the department.
  • Responsible for any insurance verification/certification needed.
  • Initiates pre-certification and recertification of insurance for all registered patients who require it.

Community – Become a wonderful addition to your local community

  • Serves as a resource person to healthcare professionals concerning psycho-social problems and community resources.

Requirements

  • Bachelor’s Degree, or higher, in Social Work or Nursing (RN or LPN) License required
  • Two or more years of previous experience in all aspects of social service or nursing with an emphasis on psychiatric work.
  • BLS
  • Handle with Care training (will be provided if not current)
  • Basic computer knowledge
  • Strong interpersonal skills
