Discharge Planner (Credentialed)

Conifer Park is looking for a Credentialed Discharge Planning expert for our Inpatient Facility For Chemical Dependency Treatment.

When Joining the Conifer Park Team, the right candidate for this role will oversee/Review all discharge plans for rehab patients for inclusion of appointments to PCP, Health Home and HARP; provide comprehensive evaluation of patient's functioning as defined by facility policy and procedures; Follow up with all AMA’s and no shows for aftercare through phone call and outreach; Offer referral to aftercare programs as needed. Documents patient progress and own interventions in case record. Needs to remain within periods of regulatory requirements. The right fit will also provide individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans in relation to discharge planning, relapse prevention and as requested by Program/Assistant director. Provides progress reports, develops aftercare plans in conjunction with referring persons/agencies, EAPs, and managed care/insurance entities while Maintains linkage agreements with other providers for aftercare services/placement. Provides comprehensive clinical assessments of patients identifying patient history, current mental health needs, psychosocial functioning, internal/external resources and concerns. Complete patient care documentation as required by Conifer Park, State and Federal regulatory bodies. Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director. Function as unit coordinator as assigned including overseeing safety issues on unit including fire drill and emergency evacuation. Collaborate with Regional Services offices and affiliated facilities to assure optimal resource utilization

Perform managed care reviews and consultations as appropriate to the role of Addiction Specialist

 
Schedules: Mondays - Fridays 8:00am - 4:30pm 

• High School Diploma, Associate’s degree or higher in a related field
• Required Certification or License: CASAC, CASAC 2, CASAC Advanced, CASAC Master, LMSW or LMHC
• 2 years’ experience in case management and discharge planning
• Proficient computer and customer service skills.
