DSP - CNA - LPN - Caregiver

Position serves individuals with developmental disabilities by working as a team member assisting with vocational work and job coaching, daily living skills, and more

DAILY TASKS TO INCLUDE:

• 
  G-Tube Changes and Catheter Changes


• 
  Medication and MAR Management/Oversight


• 
  Sliding Scale Blood Sugar Monitoring


• 
  Respiratory Treatments 


• 
  Daily Head to Toe Assessments


• 
  Follow Up Post Appointments to Ensure Compliance


• 
  Relaying Patient’s Needs to Providers


• 
  Carrying Out Physicians Orders


• 
  Identifying Needs & Initiating New Orders with Physicians


• 
  Documentation with the use of an Electronic Medical Record


• 
  Responsible for Updating Patient Information & Accurate Record Keeping


• 
  Assisting DSP with meal preparation, cleaning, and personal care.

LICENSING:

• 
  Current unrestricted Arizona PN/RN License Required


• 
  Arizona Level 1 Finger Print Clearance Card


• 
  CPR and First Aid


• 
  Article 9 Certification (required in first 90 days of hire) 


• 
  Prevention and Support Certification (required in first 90 days of hire)


• 
  Member Transportation to and from home as well as in the community
