The Elder Care Program Coordinator supports Tribal Elders by conducting home visits, assessing needs, and connecting them to medical, social, and community resources. This role serves as a liaison between Elders, their families, and service providers to ensure needs are met and services are coordinated. The coordinator maintains documentation, supports care planning, and builds relationships within the Tribal community.

The work is performed in both an office setting and in the field, including frequent travel by car to conduct home visits and provide services to Tribal Elders.
